Pelayanan kesehatan di rumah sakit harus berfokus pada mutu dan keselamatan pasien, termasuk salah satu didalamnya adalah pelayanan operasi. Pada tahun 2022 angka penundaan operasi di RSUP Fatmawati sebesar 2,3%. Secara umum adanya penundaan operasi bisa berdampak pada terjadinya cedera fisik maupun non fisik. Cedera non fisik yang terjadi adalah adanya gangguan psikologis berupa kecemasan bahkan sampai depresi. Selain itu terjadinya penundaan operasi juga berdampak pada perpanjangan lama rawat dan utility kamar operasi menjadi berkurang. Dampak dari penundaan operasi berpotensi pada terjadinya inefisiensi keuangan dimana terjadi peningkatan biaya operasional dan berkurangnya penerimaan. Tujuan dari penelitian yang dilakukan adalah untuk mengetahui adanya hubungan penundaan operasi terhadap kecemasan pasien di Rumah Sakit Umum Pusat Fatmawati Tahun 2023. Penelitian dilakukan dengan pendekatan observasional dan desain case control yang melibatkan 102 Responden pada penelitian kuantitatif dan 14 informan penelitian kualitatif. Hasil penelitian menunjukan nilai OR “Estimate” yaitu 0.183, artinya sebagai faktor protective sehingga dapat disimpulkan bahwa kejadian risiko kecemasan pasien yang mengalami penundaan operasi lebih rendah dibandingkan pasien yang tidak mengalami penundaan operasi. Tetapi p value < 0.005 menunjukkan ada hubungan yang bermakna antara penundaan operasi dengan kecemasan. Ditemukan ada hubungan kondisi pasien, hasil laboratorium dan kesiapan operator dengan penundaan operasi tetapi tidak ada hubungan signifikan antara persetujuan operasi, persetujuan penjaminan biaya dan ketersediaan ventilator terhadap penundaan operasi. Sebagai simpulannya adalah penundaan operasi berisiko menimbulkan kecemasan pasien sehingga saran penelitian ini adalah pengembangan klinik pra bedah untuk mengkaji kesiapa kondisi pasien serta meningkatkan edukasi sebelum tindakan operasi, penambahan kapasitas ruang intensive dan ventilator serta monitoring dan evaluasi.
Health services in hospitals must focus on quality and patient safety, including surgical services. In 2022 the number of postponed operations at Fatmawati General Hospital will be 2.3%. In general, delaying surgery can result in physical and non-physical injuries. The non-physical injuries that occur are psychological disorders in the form of anxiety and even depression. Apart from that, delays in surgery also have an impact on length of stay and reduced operating room utility. The impact of postponing operations has the potential to result in financial inefficiencies where operational costs increase and revenue decreases. The aim of the research carried out was to determine the relationship between postponing surgery and patient anxiety at the Fatmawati Central General Hospital in 2023. The research was carried out using an observational approach and case control design involving 102 respondents in quantitative research and 14 qualitative research informants. The research results show that the OR "Estimate" value is 0.183, meaning it is a protective factor so it can be concluded that the incidence of anxiety risk in patients who experience a delay in surgery is lower than in patients who do not experience a delay in surgery. However, p value < 0.005 indicates there is a significant relationship between delaying surgery and anxiety. It was found that there was a relationship between patient condition, laboratory results and operator readiness with surgery delays but there was no significant relationship between approval for surgery, approval for cost guarantees and ventilator availability on surgery delays. The conclusion is that postponing surgery risks causing patient anxiety, so the suggestion for this research is the development of a pre-surgical clinic to assess the patient's condition as well as increasing education before surgery, increasing the capacity of the intensive care unit and ventilator as well as monitoring and evaluation.

Tesis ini membahas mengenai budaya keselamatan pasien dan hubungannya dengan pelaporan insiden keselamatan pasien (IKP) di RSUP Fatmawati tahun 2024. Penelitian ini adalah penelitian mixed method dengan Embedded Design dimana penelitian kuantiatif dan kualitataif dilakukan bersamaan. Penelitian kuantitatif sebagai penelitian utama dan kualitatif sebagai penelitian penunjang. Hasil penelitian menunjukkan tidak ada hubungan yang signifikan antara umur, jenis kelamin, masa kerja, tingkat pendidikan, budaya keselamatan pasien dalam dimensi budaya keterbukaan, budaya keadilan dan budaya pembelajaran terhadap pelaporan insiden keselamatan pasien di RSUP Fatmawati. Tetapi masa kerja, profesi, kontak dengan pasien dan budaya pelaporan memiliki hubungan yang signifikan terhadap pelaporan insiden keselamatan pasien setelah dikontrol dengan variabel lain. Penelitian ini menyarankan rumah sakit untuk meningkatkan pemahaman karyawan mengenai keselamatan pasien dan budaya keselamatan pasien, melakukan upaya meningkatkan keberanian karyawan untuk melapor dengan tidak memberlakukan hukuman, memperbaiki sistem, memberikan umpan balik, dan melakukan evaluasi sistem penugasan atau staffing.
The focus of this study is the patient safety culture and its relationship with patient safety incident (PSI) reporting at Fatmawati Central General Hospital in 2024. This research adopts a mixed-method approach with an Embedded Design, where quantitative and qualitative research are conducted simultaneously. Quantitative research serves as the primary investigation, while qualitative research supports the main inquiry. The findings reveal no significant correlation between age, gender, length of service, educational level, patient safety culture in terms of open culture, just culture and learning culture, and PSI reporting at Fatmawati Central General Hospital. However, lenght of service, profession, patient contact, and report culture exhibit a significant relationship with PSI reporting after controlling for other variables. This study suggests hospitals enhance employee understanding of patient safety and patient safety culture, encourage employees to report incidents without fear of punishment, improve systems, provide feedback, and evaluate assignment or staffing systems.

For hospital patients, quality health services are not only felt by recovering from physical illness but also regarding patient satisfaction with the attitudes, knowledge and skills of health workers in providing services and the availability of adequate facilities and infrastructure. Patients will be satisfied if the service they receive at least equals or exceeds patient expectations. Meanwhile, the patient feels dissatisfied if the service he receives does not match the patient's expectations. Based on the achievement data of quality indicators at Bakti Timah Mentok Hospital in 2022, there are several quality indicators that have not reached the target. In addition, the number of JKN patient visits at the outpatient installation at Bakti Timah Mentok Hospital in 2020-2022 has also decreased. The purpose of this study was to describe patient satisfaction and interest in revisiting JKN patients at the outpatient installation at Bakti Timah Mentok Hospital in 2023. The study design was cross sectional with a sample size of 120 people. Statistical analysis using Chi Square test or Fisher Exact test. Based on statistical tests to see the relationship between the characteristics of the respondents and the interest in repeat visits, it was found that all of the characteristics of the respondents had p value > 0.05, which means that there was no significant relationship between the characteristics of the respondents and the interest in repeat visits. While the results of statistical tests between respondents' satisfaction on the Tangibles dimension and intention to revisit, it is known that p value > 0.05, which means that there is no significant relationship between respondent's satisfaction on the Tangibles dimension and intention to revisit. And the results of statistical tests between respondents' satisfaction on the dimensions of Reliability, Responsiveness, Assurance and Empathy with interest in repeat visits are known to be p value ≤ 0.05, which means that there is a significant relationship between respondents' satisfaction in the dimensions of Reliability, Responsiveness, Assurance and Empathy with interest in repeat visits. The relationship between patient satisfaction as a whole resulted in p value ≤ 0.05, which means that there is a significant relationship between patient satisfaction and interest in repeat visits. The results of the mapping of service elements in the Cartesian diagram show that 5 (five) service elements are in quadrant A, which means they are a top priority for improvement efforts. The five elements of service are the condition of the patient's waiting room, the availability of medical devices, the availability of types of laboratory examinations, the availability of drugs in the hospital pharmacy installation and the presence of doctors according to the service schedule. Therefore, the Bakti Timah Mentok Hospital must pay more attention to service elements that have not provided maximum satisfaction to patients.

Bagi pasien rumah sakit, pelayanan kesehatan yang berkualitas tidak hanya dirasakan dengan sembuhnya dari penyakit secara fisik namun juga menyangkut kepuasan pasien terhadap sikap, pengetahuan dan keterampilan tenaga kesehatan dalam memberikan pelayanan serta tersedianya sarana dan prasarana yang memadai. Pasien akan puas apabila layanan yang diterimanya setidaknya sama atau melampaui harapan pasien. Sedangkan pasien merasa tidak puas apabila layanan yang diterimanya tidak sesuai dengan harapan pasien. Berdasarkan data capaian indikator mutu di Rumah Sakit Bakti Timah (RSBT) Mentok Tahun 2022, terdapat beberapa indikator mutu yang belum mencapai target. Selain itu angka kunjungan pasien JKN di instalasi rawat jalan RSBT Mentok Tahun 2020-2022 juga mengalami penurunan. Tujuan penelitian ini adalah untuk mengetahui gambaran kepuasan pasien dan gambaran minat kunjungan ulang pada pasien JKN di instalasi rawat jalan RSBT Mentok Tahun 2023. Desain penelitian ini adalah Cross Sectional dengan jumlah sampel sebanyak 120 orang. Analisis statistik menggunakan uji Chi Square atau uji Fisher Exact. Berdasarkan uji statistik untuk melihat hubungan karakteristik responden dengan minat kunjungan ulang didapat hasil seluruh karakteristik responden memiliki p value > 0,05 yang artinya tidak ada hubungan yang signifikan antara karakteristik responden dengan minat kunjungan ulang. Sedangkan hasil uji statistik antara kepuasan responden pada dimensi Tangibles dengan minat kunjungan ulang diketahui p value > 0,05 yang artinya tidak ada hubungan yang signifikan antara kepuasan responden pada dimensi Tangibless dengan minat kunjungan ulang. Dan hasil uji statistik antara kepuasan responden pada dimensi Reliability, Responsiveness, Assurance dan Emphaty dengan minat kunjungan ulang diketahui p value ≤ 0,05 yang artinya ada hubungan yang signifikan antara kepuasan responden pada dimensi Reliability, Responsiveness, Assurance dan Emphaty dengan minat kunjungan ulang. Hubungan kepuasan pasien secara keseluruhan didapat hasil p value ≤ 0,05 yang artinya ada hubungan yang signifikan antara kepuasan pasien dengan minat kunjungan ulang. Hasil pemetaan unsur pelayanan pada diagram Kartesius didapat hasil 5 (lima) unsur pelayanan berada pada kuadran A yang artinya menjadi prioritas utama untuk dilakukan upaya perbaikan. Kelima unsur pelayanan tersebut yaitu kondisi ruang tunggu pasien, ketersediaan alat kesehatan, ketersediaan jenis pemeriksaan di laboratorium, ketersediaan obat di instalasi farmasi rumah sakit dan kehadiran dokter sesuai jadwal pelayanan. Oleh karena itu pihak RSBT Mentok harus lebih memperhatikan unsur-unsur pelayanan yang belum memberikan kepuasan maksimal kepada pasien.

Lamanya waktu tunggu pelayanan pasien pulang rawat inap di RSUD KiSA Kota Depok mengakibatkan lamanya perpindahan pasien dari IGD yang perlu dilakukan rawat inap. Hal ini merupakan alasan pentingnya pemantauan proses pemulangan pasien yang selama ini belum pernah dilakukan, dan perlunya ditentukan target waktu tunggu proses pemulangan pasien rawat inap. Tujuan penelitian adalah mengetahui implementasi Lean Six Sigma pada proses pasien pulang rawat inap dalam upaya perbaikan mutu pelayanan rawat inap di RSUD KiSA, dengan tahapan Define, Measure, Analyze, Improve dan Control berfokus pada penurunan aktivitas non value added serta pemborosan dengan meningkatkan aktivitas value added. Penelitian ini menggunakan pendekatan kualitatif dengan desain action research, menggunakan metode wawancara mendalam, observasi dan telaah dokumen, serta implementasi perbaikan proses pemulangan pasien rawat inap. Hasil penelitian menunjukkan adanya perbaikan waktu tunggu pelayanan pasien pulang rawat inap. Kondisi awal proses pemulangan pasien diketahui ratio VA hanya 23,9% dan ratio NVA sebesar 76,1% dengan Lead Time 315,08 menit. Setelah dilakukan pendekatan Lean Six Sigma dapat diidentifikasi pemborosan dan dilakukan upaya perbaikan. Sehingga terjadi penurunan Lead Time menjadi 192,50 menit dengan peningkatan rasio VA menjadi 30,6%. Rekomendasi dari penelitian ini adalah perlunya penetapan alur proses pemulangan pasien serta dilakukan pemantauan waktu tunggu melalui indikator mutu unit rawat inap. Proses evaluasi dilakukan untuk menilai mutu layanan yang diberikan sudah memberikan manfaat bagi pasien dan operasional pelayanan rumah sakit.
The long waiting time for inpatient services at the KiSA Hospital in Depok City resulted in a long transfer of patients from the emergency room who had to be treated at the hospital. This is the reason for the importance of monitoring the patient discharge process which has never been done before, and the need to set a target waiting time for the inpatient discharge process. The research objective was to determine the application of Lean Six Sigma to the inpatient discharge process in an effort to improve the quality of inpatient services at KiSA Hospital, with the Define, Measure, Analyze, Improve and Control stages which focus on reducing non-value, adding activity and waste by increasing value added activity. . This study used a qualitative approach with an action research design, using in-depth interviews, observation and document review, as well as implementing improvements to the inpatient process. The results showed an increase in waiting time for inpatient discharge services. The initial condition of the patient discharge process was that the VA ratio was only 23,9% and the NVA ratio was 76,1% with a lead time of 315,08 minutes. After taking the Lean Six Sigma approach, waste can be identified and efforts to fix it can be made. So that there is a decrease in Lead Time to 192,50 minutes with an increase in the VA ratio of 30,6%. The recommendation from this study is the need to determine the flow of the patient discharge process and monitor waiting time through inpatient unit quality indicators. The evaluation process is carried out to assess the quality of services provided which have benefited patients and hospital service operations.

Tesis ini membahas implementasi pelayanan berorientasi pasien di Rumah Sakit Umum Tangerang Selatan dan hubungannya dengan persepsi tenaga kesehatan tentang dukungan organisasi yang dirasakan oleh tenaga kesehatan. Penelitian ini adalah penelitian kuantitatif dengan desain cross sectional. Hasil penelitian menyarankan bahwa manajemen Rumah Sakit Umum Tangerang Selatan untuk terus meningkatkan pelayanan berorientasi pasien khususnya pada dimensi preferensi pasien, dan keterlibatan keluarga dalam rencana perawatan pasien; membangun sistem reward bagi tenaga kesehatan yang transparan; menyediakan fasilitas platform penyampaian saran perbaikan baik online maupun offline; memfasilitasi tenaga kesehatan dengan pelatihan manajemen stress dan komunikasi efektif dan empatik dalam menunjang pelaksanaan pelayanan berorientasi pasien yang lebih baik.

Penilaian budaya keselamatan pasien adalah satu elemen dasar dalammeningkatkan upaya keselamatan pasien. Sangat penting untuk menilaibagaimana sikap, persepsi, kompetensi individu dan perilaku orang / kelompoksehingga menentukan komitmen dalam meminimalkan insiden. Penelitian itubertujuan untuk mengetahui faktor-faktor yang berhubungan dengan budayakeselamatan pasien. Jenis penelitian ini menggunakan metode crossectional padapopulasi seluruh pegawai Rumah Sakit Pusat Otak Nasional (RSPON) berjumlah649 orang. Pengambilan sampel menggunakan teknik simple random samplingdengan besar sampel 250 orang responden. Analisis data yang dilakukan adalahanalisa regresi logistik ganda .Hasil penelitian menunjukkan bahwa budaya keselamatan pasien diRSPON memiliki respon positif 46,8%. dalam kategori budaya kurang kuat.Telah terbukti secara statistic bahwa terdapat hubungan signifikan dengan budayakeselamatan pasien Harapan dan Tindakan Promosi Keselamatan Pasien ,Persepsi Keselamatan Pasien, Komunikasi Terbuka, Pembelajaran Organisasi,Komitmen Manajemen, Umpan balik insiden, Kerjasama didalam unit dankerjasama diantara unit kerja, serta Serah terima dan pergantian shift. Faktor yangdominan berhubungan dengan budaya keselamatan pasien di RSPON tahun 2016adalah faktor pekerjaan yaitu Serah terima dan pergantian shif serta faktororganisasi berupa Manajemen SDM/staffing dan faktor pekerjaan yaitu Kerjasamaantar unit yang secara signifikan memiliki peluang untuk budaya keselamatanpasien yang lebih baik.Disarankan agar RSPON mengembangkan budaya keselamatan pasiendengan upaya peningkatan mutu dan keselamatan pasien. Mmengevaluasiinstrument atau dokumentasi serah terima pasien atau pekerjaan untukkeselamatan pasien. The dominant factoraffecting patient safety culture are job factor that handoff or transition and shift(p = 0.03) and cooperation between working units (p = 0.035), as well asorganizational factors, namely: staffing (p = 004). It is recommended to responseimprove patient safety culture (a) develop instruments handover of patients orwork for the safety of the patient, (b) development and evaluation procedure theinformation on the handover mainly verbal orders and telephones, (c) optimizingthe turn of the job / shift through changes in work shifts a maximum of 3 days andestablish effective communication as well as their supervision in patient safety, (d)identify a model of teamwork between the units and improve the coordination ofteamwork between units.Keywords: Patient Safety Culture, factor person, system, job

Abstrak:
Kepuasan pasien merupakan salah satu indicator dalam mengukur mutu pelayanan di rumah sakit dan juga sebagai indikator standard suatu rumah sakit. Salah satu cara atau metode yang dianggap cukup objektif dalam mengukur kepuasan pasien adalah Metode Servqual (Service Quality), ada 5 (lima) dimensi mutu untuk mengukur kepuasan pasien yaitu tangible, reliability, responsiveness, assurance, dan empathy. Tujuan penelitian ini untuk menganalisis faktor faktor yang berhubungan dengan kepuasan pasien di RS PON. Penelitian menggunakan pendekatan kuantitatif desain cross sectional study. Sampel adalah pasien rawat jalan sebanyak 200 orang, pengumpulan data dilakukan melalui kuesioner. Hasil penelitian yaitu nilai rata-rata kepuasan pasien adalah 64%. Hasil penelitian menunjukan adanya hubungan antara variabel umur dengan p value 0,026, variabel jenis kelamin dengan p value 0,001, variabel pendidikan dengan p value 0,001, variabel cara bayar dengan p value 0,036, variabel pelayanan administrasi dengan p value 0,003, variabel pelayanan perawat dengan p value 0,003, variabel pelayanan dokter dengan p value 0,001 dan variabel penunjang dengan p value 0,001 dengan kepuasan pasien. Karakteristik pasien yang merasa puas dengan pelayanan di instalasi rawat jalan RS. PON adalah pasien yang berumur tua, berjenis kelamin perempuan, pendidikan tinggi,cara bayar BPJS, yang mendapatkan pelayanan administrasi baik, yang mendapatkan pelayanan perawat baik, yang mendapatkan pelayanan dokter baik dan pelayanan penunjang baik. Secara keseluruhan kepuasan pasien di instalasi rawat jalan RS. PON masih rendah karena 90%.

Loyalitas pasien adalah salah satu topik yang berkembang dalam literatur layanan kesehatan karena telah terbukti dapat memberikan banyak manfaat, seperti peningkatan keuntungan dan komunikasi mulut ke mulut (word of mouth) yang positif. RSU HGA merupakan rumah sakit swasta yang berada di Kota Depok dan berdampingan dengan beberapa rumah sakit lain sebagai kompetitor. Jumlah pasien rawat jalan di RSU HGA mengalami penurunan dari tahun 2019 dan mulai meningkat pada tahun 2023 tetapi belum seperti tahun 2018. Desain penelitian cross sectional dengan jumlah sampel 200 responden dari populasi di unit rawat jalan RSU HGA. Ada beberapa variabel yang diteliti yaitu aksesibilitas, kepuasan, switching cost, kualitas, sikap relatif pasien yang dihubungkan dengan minat memberikan rekomendasi word of mouth pada masyarakat. Analisis data dilakukan dengan analisis univariat (deskriptif) dan multivariat dengan SEM PLS. Dari analisis data didapatkan seluruh konstruk dinyatakan valid dan reliabel sesuai dengan perhitungan loading factor, AVE dan cronbach alpha. Uji hipotesis menunjukkan variabel switching cost dan kualitas pelayanan berhubungan sikap relatif, serta sikap relatif berhubungan terhadap word of mouth. Sedangkan variabel lainya ditolak yaitu aksesibilitas, kepuasan dan faktor situasional tidak berhubungan terhadap sikap relatif. Dengan mediasi sikap relatif, aksesibilitas, kepuasan dan faktor situasional tidak berhubungan terhadap word of mouth sedangkan kualitas pelayanan dan switching cost berhubungan. Dengan adanya penelitian ini, maka sebaiknya RSU HGA melakukan peningkatan program loyalitas pasien yang sesuai dengan kebutuhan dan keefektifan pelayanan kesehatan.
Patient loyalty is one of the growing topics in healthcare literature as it has been shown to provide many benefits, such as increased profits and positive word of mouth. RSU HGA is a private hospital located in Depok City and adjoins several other hospitals as competitors. The number of outpatients at HGA Hospital has decreased from 2019 and began to increase in 2023 but not yet like 2018. Cross sectional research design with a sample size of 200 respondents from the population in the outpatient unit of HGA Hospital. There are several variables studied, namely accessibility, satisfaction, switching costs, quality, relative patient attitudes related to interest in providing word of mouth recommendations in the community. Data analysis was carried out by univariate (descriptive) and multivariate analysis with SEM PLS. From the data analysis, it was found that all constructs were declared valid and reliable according to the calculation of the loading factor, AVE and Cronbach alpha. Hypothesis testing shows that switching cost and service quality variables are related to relative attitudes, and relative attitudes are related to word of mouth. While other variables are rejected, namely accessibility, satisfaction and situational factors are not related to relative attitudes. With the mediation of relative attitudes, accessibility, satisfaction and situational factors are not related to word of mouth while service quality and switching costs are related. With this research, HGA Hospital should improve patient loyalty programmes that are in accordance with the needs and effectiveness of health services.
