Memasuki abad ke 21 yang semakin maju, agar tetap eksis di tengah persaingan global yang sernakin ketat, sudah seharusnyalah bahwa pendekatan mutu layanan yang berorientasi pada pelanggan atau pasien menjadi strategi utama bagi pelayanan kesehatan di Indonesia. Salah satu langkah strategis yang digunakan adalah kerjasama lira, disamping peningkatan mutu secara terus menerus dan pengambilan keputusan berdasarkan data yang ada. Puskesmas merupakan salah satu organisasi pelayanan kesehatan, mempunyai beban kerja 18 program pokok yang terbagi atas kegiatan-kegiatan, dikelola oleh seorang pemimpin bersama staf yang terdiri dari berbagai latar belakang. Mengkoordinir individu yang terdiri dari berbagai latar belakang ini bukanlah pekerjaan mudah, agar setiap saat tidak terjadi konflik. Dalam studi ini dilakukan penelitian tentang analisis kerjasama tim di Puskesmas wilayah Kota Pontianak tahun 2000, untuk mendapatkan informasi tentang gambaran kerjasama tim dan hal-hal yang berperan dalam kerjasama tim tersebut. Penelitian ini menggunakan metode penelitian kualitatif yang dilakukan terhadap 4 (empat) Puskesmas dengan pengunjung terbanyak, dan 4 (empat) Puskesmas dengan pengunjung paling sedikit, dengan melibatkan 8 orang Kepala Puskesmas dan 1 orang Kepala Dinas melalui wawancara mendalam, dan 28 orang staf dari Puskesmas tadi melalui diskusi kelompok terarah. Analsisa dan data yang terkumpul menunjukan bahwa gambaran kerjasama tim wilayah Kota Pontianak cukup baik, hal ini terlihat dari karateristik individu seperti jenis kelamin, suku bangsa dan jabatan tidak menjadi masalah dalam kerjasama tim, namun disisi lain karakteristik seperti umur, pendidikan, status perkawinan, dan lama kerja disamping dapat menunjang kerjasama tim, juga dapat menghambat kerjasama tim. Begitu juga dengan karateristik organisasi. Hal-hal yang berperan dalam kerjasama tim berdasarkan variabel karakteristik organiosasi secara negatif antara lain, untuk Puskesmas besar (dan segi jumlah kunjungan dan tingginya kesibukan) menyebabkan berkurangnya intensitas interaksi, walaupun sudah semua Puskesmas mempunyai visi dan misi, tetapi tidak satupun balk staf maupun pimpinan yang tabu dan ingat isi dari visi dan misi tersebut. Sementara karateristik organisasi yang berperan secara positif yaitu, komunikasi secara informal yang berdampak lebih akrab, kepemimpinan secara demokrasi dengan mengutamakan pelayanan kepada masyarakat, rasa tanggung jawab dalam bekerja yang memotivasi staf untuk bekerja lebih balk, adanya rasa kekeluargaan yang kuat sehingga walaupun ada konflik tetapi tidak menghambat kerjasama tim. Berdasarkan hat tersebut diatas maka untuk membangun kerjasama tim yang tangguh diperlukan sating toleransi terhadap keragaman latar belakang anggota, memanfaatkan pertemuan-pertemuan formal untuk membahas hat-hat yang berkaitan dengan upaya perbaikan mute layanan kesehatan, selain itu mengadakan pertemuan informal seperti arisan atau anjang sana untuk meningkatkan kualitas hubungan antar anggota. Selanjutnya agar konflik tidak menghambat kerjasama tim, maka rasa kekeluargaan perlu dibina serta pihak Kepala Puskesmas cepat tanggap untuk menanganinya. Kepada Kepala Dinas Kesehatan Kota Pontianak, diharapkan untuk melengkapi sarana komunikasi dan transportasi di Puskesmas karena sarana tersebut sangat menunjang kerjasama tim. Perlu penyegaran informasi tertang visi dan misi agar tim memiliki kesatuan tujuan dalam bekerja.
Analysis on Teamwork in Improving Puskesmas Service Quality in Pontianak City Area Year 2000 Entering the 21st century, to stay exist amid tight global competition, health service in Indonesia should be provided on the basis of customer or patient-oriented service that becomes a main strategy in Indonesia health service. One of strategic ways to use in the service is teamwork, other ways include continuous quality improvement and decision making based on the available data. Puskesmas is one of health service organizations. It has 18 main tasks divided into programs and is managed by a chief assisted with staffs from various disciplines. Coordinating these staffs with various backgrounds is not an easy task, particularly to prevent conflicts among them. This study examined teamwork in Puskesmas in Pontianak City area year 2000. It was aimed at obtaining information regarding description of teamwork and factors that affect such teamwork. This study employed a qualitative research approach and was conducted in 4 (four) most-attended Puskesmas, and 4 (four) least-attended Puskesmas. It involved 8 chiefs of Puskesmas and 1 chief of local health department. Data were collected by means of in-depth interviews with the 8 chiefs of Puskesmas and 1 chief of local health department and focused group discussion with 28 staff members of the Puskesmas. The analysis on the collected data shows that the description of teamwork in Puskesmas in Pontianak City area is good enough. Gender, ethnic group and position were not obstacles in the teamwork. However, age, education, marital status, and length of work as well as the organization characteristics seemed to be both supporting and hindering the teamwork. Factors that seem to have negatively affected the teamwork based on the variables of organization characteristics among others are lack of intense interaction due to the size of Puskesmas or in larger Puskesmas (in terms of the number of visits and rate of service); although all Puskesmas had already set their vision and mission statements but none of the staffs nor the superiors were able to recall such statements. On the other hand, organization characteristic that seem to have positively affected the teamwork are informal communication that brings members closer to each other, democratic leadership that employs public-oriented service, work responsibility that motivates the staffs to do their jobs better, strong family hood that ties the members of the team despite present conflicts. Based on such findings, to build a solid teamwork, tolerance over differences of members is required, formal meetings to discuss issues regarding improvement of health service quality are utilized, and informal meetings such as family gathering or home visits are encouraged to maintain and improve relationship among team members. To prevent conflict from developing, it is necessary to foster sense of family hood and chief of Puskesmas should be able to respond to any conflict immediately. Chief of local health department of Pontianak City is suggested to provide communication and transportation equipment for Puskesmas that may support the teamwork. Reorientation program on vision and mission of puskesmas should be held so those members of team share one common goal in doing their job.
This study is quantitative research using cross sectional design to know the relationship of characteristic and job satisfaction on performance of Public Health Care officer in Belitung Regency in 2017. The sample in this research is civil servant which is found in 9 (nine) PHC in Belitung Regency which amounts to 180 people. The results showed that the characteristic variables related to the performance of PHC employee in Belitung Regency were the last education. Job satisfaction variable related to performance of PHC employee of Belitung Regency is compensation. The variable of characteristics and job satisfaction are related after tested together (simultaneous) there are three significant variables are compensation, relationship with colleagues and supervision. DKI Jakarta Province has several health challenges, one of which is Triple Burden Disease, in which Communicable Diseases (CD) remind high, then Non Communicable Diseases (NCD) are increasing, besides Emerging Infectious Diseases (EID)/ ReEmerging and/ or New Emerging. According to the Republic of the Indonesia Ministry of Health, PIE got special attention due to its serious impact on health and socioeconomics, particularly in the current digital era and globalization. Along the development of the situation and conditions of the pandemic, the Emergency Response Status for the COVID-19 Outbreak of the DKI Jakarta Province in 2020 requires innovation in improving the quality of public services and health care. DKI Jakarta Province has received many awards, one of which is the most innovative province. However, innovations in the health sector that are include in the top of 99 public service innovations are only 2%. Therefore, an in-depth analysis is needed regarding innovations in the health sector on the quality of services during the COVID-19 Pandemic. This type of research is mix method by using a combination type Sequential Explenatory. The independent variables include leadership, innovation culture, resource training, communication channels, networks and partnerships, rewards, complexity and relative advantage, perceived usefulness, and perceive ease of use, as well as the dependent variable consisting of aspects of quality with a structure, process, and output approach. The research was conducted at Community Health Center and DKI Jakarta Health Office in Mei-June 2021. The research locations in The Community Health Center in 5 Regencies at DKI Jakarta Province. Data analysis used univariate, bivariate (Chi Square) and multivariate with logistic regression. The result showed that the quality of health services during the pandemic was quite good about 71,8%. The results of the analysis found that there was a relationship between the factors of implementating innovation and the utilization of information technology in the health sector on the quality of services during the COVID-19 pandemic at the DKI Jakarta Provincial Health Center for the 2020-2021 Period are leadership, innovation culture, network and partnership, and reward. The most dominant variable is leadership which is interact with reward with OR value 7,64.
Pada era globalisasi dan persaingan bebas dalam bidang pelayanan kesehatan saat ini pihak pengelola pelayanan kesehatan dituntut untuk meningkatkan kualitas pelayanannya. Di Kabupaten Lampung Utara pemanfaatan puskesmas sebesar 45,2%. Oleh Yudha,dkk, di Puskesmas Banjit tahun 1997 menghasilkan angka dengan kepuasan pasien sebesar 87%. Survai pendahuluan dilakukan bulan Maret 2002 di 4 puskesmas didapatkan 68% responden tidak puas terhadap layanan petugas. Sayang sekali data diatas, belum secara spesifik menggali tentang data kepuasan pasien terhadap layanan kesehatan di Kabupaten Lampung Utara.Penelitian ini bertujuan untuk mengetahui bagaimana tingkat kepuasan dan mutu layanan balai pengobatan serta bagaimana hubungan kepuasan dan mutu layanan setelah dlcontrol dengan karakteristik pasien, yaitu : jenis kelamin, pendidikan, pekerjaan dan pengetahuan. Penelitian ini dilakukan melalui analisis kuantitatif, dan pendekatan cross sectional untuk menganalisa korelasi antara faktor penyebab dan akibat melalui observasional-pengumpulan data sekaligus pada suatu saat pada 378 pasien balai pengobatan dari tanggal 22-25 April 2602 di 17 puskesmas Kabupaten Lampung Utara, guna mengetahui tingkat kepuasan pasien rawat jalan pada tahun 2002. Untuk memperoleh alasan ketidakpuasan yang timbul akibat proses pelayanan digunakan pendekatan kualitatif.Hasil yang didapat menunjukkan tingkat ketidak kepuasan pasien sebesar 48,7%. Tingkat mutu layanan dimana yang menyatakan tidak ada tanggung jawab manajemen sebanyak 80,2%, sedangkan tidak ada proses penyerahan jasa sebanyak 50,5%. Pada uji bivariat dengan chi-quare didapat terdapat hubungan signifikan antara mutu layanan dengan tingkat kepuasan pasien balai pengobatan puskesmas di Kabupaten Lampung Utara tahun 2002, dimana pada faktor tanggung jawab manajemen variabel yang merupakan konfonder adalah pendidikan dan pekerjaan, sedangkan pada faktor proses penyerahan jasa variabel konfondennya jenis kelamin, pendidikan dan pekerjaan. Pada penelitian ini didapat indikator kepuasan yang diprioritaskan adalah : keterampilan pemeriksa, menerangkan cara minum obat yang akan diberikan, mengajak bila pasien sakit untuk berobat kembali.Disarankan pada pembuat kebijakan pemerintah daerah dalam hal ini pihak dinas kesehatan Kabupaten agar : Mengadakan simulasi aplikasi pemeriksaan pasien sesuai dengan protap pelayanan standar minimal balai pengobatan puskesmas yang telah dibuat oleh Dinas kesehatan Kabupaten, mengadakan penyegaran bagi petugas balai pengobatan tentang perjalanan penyakit dan penatalaksanaan pengobatan sederhana serta tata cara pemberian obat-obatan di puskesmas, mengadakan penyegaran penyusunan program kerja puskesmas yang berorientasi pada kepuasan pelanggan, dan mengadakan survei berkala untuk pengukuran kepuasan pasien dengan periode waktu tertentu (misalnya setiap akhir tahun anggaran), sehingga diketahui faktor-faktor yang mempengaruhi sehingga dapat digunakan sebagai batas perencanaan peningkatan mutu layanan dimasa yang akan datang.
Relationship between Service Quality and Patient Satisfaction at Puskesmas in the District of North Lampung 2002In the recent globalization and free-trade era, the health care manager requires improving the service quality in the health care. Community Health Center (Puskesmas) utilization in the District of North Lampung is 45,2%. Yudha et.al revealed that patient satisfaction in Puskesmas Banjit in 1997 was 87%. The preliminary survey that conducted in March 2002 in 4 Puskesmas showed that 68% of respondents not satisfied with the provider's service. Unfortunately, the data that mentioned above was not specifically explained yet about the patient satisfaction on service quality at Puskesmas in the District of North Lampung.The aims of this study were to find the level of satisfaction among patients and the service quality of health center as well as to find the relationship between patient satisfaction and service quality after being controlled by characteristics of patients such as sex, education, occupation, and knowledge. This study used the quantitative analysis and cross sectional approach to analyze relationship between causal factor and effect factor through observation. Data collecting was conducted to 378 patients of health center on April 22-25, 2002 at 17 Puskesmas in the District of North Lampung. To gain the reasons among unsatisfied patients on the service process given, this study used the qualitative approach.The results of this study showed that the level of satisfaction among patients was 48,7%. Based on the level of service quality stating the lack of management responsibility was 80,2%, while no availability of the process of service delivery was 50,5%. A Chi-square test revealed the availability of significance between service quality and the level of patient satisfaction at Puskesmas in the District of North Lampung. As confounder variables in the management responsibility factor were education and occupation, while confounder variables in the process of service delivery factor were sex, education, and occupation. This study also found that the indicators of satisfaction having priority to be done are health provider's skill, explanation how to take the medicines, and suggest doing the examination again if the patient were sick.The recommendations for the policy maker of district government, especially for the Health District, are to conduct simulation of examination for patient based on the minimal standard operational procedure of Puskesmas, to review Puskesmas program planning that focused on the customer satisfaction, and to conduct a periodic survey (at the end of the fiscal year) so it could determine the factors related to the patient satisfaction. And it is hoped that they could be used to improve the service quality in the future.
Tesis ini membahas hubungan antara persepsi dimensi kualitas pelayanan kesehatan dengan kepuasan pasien Askes di Puskesmas Sukasari di Kota Tangerang. Penelitian ini dilakukan karena peneliti melihat adanya penurunan kunjungan pasien Askes pada tahun 2012. Penelitian ini merupakan penelitian observasional dengan desain longitudinal. Teori yang digunakan untuk melihat dimensi kualitas adalah teori Cronin and Taylor (1992) yang terdiri dari lima variabel yaitu tangible, reliability, responsiveness, assurance, dan emphaty. Hasil penelitian menunjukkan bahwa dimensi kualitas yang signifikan berhubungan dengan kepuasan adalah tangible, reliability, responsiveness, assurance, dan emphaty dimana tangible adalah variabel dimensi kualitas yang paling kuat hubungannya dengan kepuasan.
Berdasarkan hasil penelitian, peneliti menyarankan agar puskesmas meningkatkan kenyamanan pasien dengan meningkatkan fasilitas yang ada, meningkatkan kedisiplinan jadwal pelayanan, meningkatkan kesigapan dokter dan perawat, serta meningkatkan keterampilan perawat. Dinas Kesehatan perlu upaya peningkatan pembinaan kedisiplinan dan meningkatkan pelayanan puskesmas Sukasari menjadi rawat inap serta PT Askes perlu upaya peningkatan pelayanan dengan memberikan informasi hak dan kewajiban peserta Askes dan memfasilitasi program-program pencegahan di puskesmas lain di Kota Tangerang.
This thesis explores the relationship between perceptions of the dimensions of quality of health care at the health center patient satisfaction Askes Sukasari Tangerang City. This research was conducted because researchers saw a decrease in patient visits in 2012 Askes. This study is an observational study with a longitudinal design. Theory that used to see the dimensions of quality is the theory of Cronin and Taylor (1992) which consists of five variables: tangible, reliability, responsiveness, assurance, and empathy. The results showed that the quality dimension significantly related to satisfaction is tangible, reliability, responsiveness, assurance, empathy and tangible is the variable dimension quality where the most powerful relationship with satisfaction.
Based on this study, the researcher suggests the health center to improve their Askes patients? satisfaction through their Public Health Care Services by improving their facility, improving discipline in their service schedule, improving the responsiveness of the doctors and nurses, and improving the skills of their nurses. Diantara tahun 2002 - 2005, terjadi konflik/unjuk rasa staf di 7 Puskesmas di wilayah Kabupaten Bekasi. Berdasarkan survei awal yang dilakukan penulis untuk membuktikan dugaan penyebab terjadinya konflik, ternyata tingkat kepuasan kerja Staf Puskesmas masih rendah, yaitu hanya 56,17%, serta adanya hubungan yang positif antara kepemimpinan dengan kepuasan kerja Staf Puskesmas. Survei yang sejenis belum pernah dilakukan di Kabupaten Bekasi. Penelitian ini bertujuan membuktikan adanya hubungan kepemimpinan transfomasional, transaksional dan laissez faire terhadap tingkat kepuasan kerja Staf Puskesmas di 34 Puskesmas yang berada di wilayah Kabupaten Bekasi. Desain penelitian merupakan penelitian analitik deskriptif dengan pendekatan cross sectional, yang dilakukan di seluruh Puskesmas di Kabupaten Bekasi. Data dikumpulkan dari seluruh Staf Puskesmas yang berjumlah 681 orang dan 33 Kepala Puskesmas pada tahun 2006. Analisis data menggunakan analisis univariat dan bivariat dengan menggunakan uji Chi Square dan uji korelasi sedehana. Hasil penelitian menunjukkan bahwa tingkat kepuasan kerja Staf Puskesmas sebesar 2,9% (dengan cut-off point 80%) dan 49,6%(dengan cut-off point nilai tengah). Peralatan kerja Puskesmas merupakan faktor kepuasan kerja Staf yang mempunyai nilai terendah. Mayoritas Kepala Puskesmas menampilkan kepemimpinan transformasional yang dikombinasikan dengan kepemimpinan transaksional. Ada hubungan yang bermakna antara dimensi attributed charissmatik dari kepemimpinan transformsional dengan kepuasan kerja Staf Puskesmas. Staf Puskesmas yang mempersepsikan dimensi attributed charismatic Kepala Puskesmasnya "kuat" mempunyai peluang untuk merasa lebih puas dalam bekerja. Hubungan ini mungkin masih dipengaruhi oleh umur, jenis kelamin, tingkat pendidikan, latar belakang profesi Staf Puskesmas, serta besamya insentif dan tunjangan hari raya yang diterima. Dimensi attributed charismatik dari kepemimpinan transformasional mampu meningkatkan kepuasan kerja Staf Puskesmas. Perlunya pengenalan tipe kepemimpinan ini kepada para Kepala Puskesmas. Hal ini dapat digunakan dalam seleksi calon Kepala Puskesmas dan pelatihan-pelatihan kepemimpinan bagi orang¬-orang yang akan dipromosikan menjadi pimpinan dalam organisasai kerjanya. Peralatan kerja Puskesmas yang mempunyai skor kepuasan terendah, perlu menjadi perhatian semua pihak yang terkait.
By the years of 2002 - 2005 there were conflicts/demonstrations in 7 Public Health Centers in Bekasi district. Based on preliminary survey conducted by the author to proved that the assumption of conflict caused, the level of job satisfaction among Public Health Center staffs was low, i.e. only 56,17%, and there was positive correlation between leadership and job satisfaction of Public Health Center staffs. A similar survey has not been conducted yet in Bekasi district. This survey attempts to prove the existence of the correlation between transformational, transactional and laissez faire leadership to the level of job satisfaction in 34 Public Health Centers in Bekasi district. The Research design was a descriptive-analytic research with cross-sectional approach, which carried out to all Public Health Centers in Bekasi district. Data were collected from all 681 staffs and 33 Public Health Center heads in 2006. These were analyzed by using univariat and bivariat analyses with Chi Square and simple corelation test. The study proved that respondents with job's satisfaction only 2,9% (with cut-off point 80%) and 49,6% (with mid-value cut-off point). The instrument of Public Health Center was a job-satisfaction factor with the lowest value. The majority of Public Health Center heads performed a transformational leadership combined with transactional one. There was an important correlation between attributted charismatic dimension of transformational leadership with employee job-satisfaction. Public Health Center staffs which have a perception about their Public Health Center head's attributed charismatic dimension as strong, have a chance to feel more satisfied in their job. This perhaps was influenced by the factors of age, sex, education and profession background of Public Health Center staff, and the amount of incentives and THR. Attributed charismatic dimension of transformational leadership tends to increase the employee's job-satisfaction. It is necessary to introduce this type of leadership to the head Public Health Centers. This can be applied in the selection of the Public Health Center head candidates, and leadership training for those that will be promoted to be a leader in their work organization. It is necessary for all related sides to pay attention to the instrument of Public Health Center that has the lowest value in job-satisfaction score.
