ABSTRAK Aktivitas rig up lokasi well service PT. Pertamina EP Region Jawa Field Jatibarang dapat berpotensi terjadi kecelakaan berupa kerusakan peralatan rig, maupun fatality pada manusia yang dilakukan oleh operator rig. Tujuan dari penelitian ini adalah untuk mengetahui probabilitas terjadinya human error dan violation pada aktivitas rig up yang dilakukan oleh operator rig. Skill, rule dan knowledge base error merupakan klasifikasi yang dipengaruhi oleh jenis informasi yang terlibat dalam kegiatan industri yang dikembangkan oleh J Rasmussen. Menurut Reason, terjadinya tindakan violation adalah keputusan secara sadar, sedangkan error terjadi terlepas dari kehendak seseorang untuk menghindari. Probabilitas terjadinya potensi human error dan violation yaitu ketika operator rig melakukan tahapan task yang salah selain standard operating procedure. Sekitar 63,9% probabilitas rule based error dari 111 tahapan task rig up. Bagi perusahaan lakukan training, pengawasan, review standard operating procedure, dan inspeksi. Bagi operator rig meningkatkan komunikasi dua arah secara jelas antara operator rig dan sinyalman. Bagi peneliti lain sebagai referensi melakukan kajian error dan violation pada rig pemboran.
ABSTRACT Rig up Activity on well service locations PT. Pertamina EP Region Jawa Field Jatibarang could potentially accident rig equipment damage, or fatality in humans conducted by the rig operator. The purpose of this study is to investigate the probability the potential of human error and violation rig up on the activities carried out by the rig operator. Skill, rule and knowledge base classification error is influenced by the type of information involved in industrial activities developed by J. Rasmussen. According to Reason, the act of violation is a conscious decision, while the error occurs regardless of the will of a person to avoid. Probability the potential of human error and the potential violation is when the rig operators did the wrong stage of the task in addition to standard operating procedure. Approximately 63.9% chance of error of rule-based task rig up 111 steps. For companies doing training, supervision, review standard operating procedure, and inspection / standardization. For rig operators improve two-way communication between the operator clearly and signalman rig. As a reference for other researchers to study error and violation on the drilling rig.
Risk analysis for 6” crude oil pipeline system SP PDT I - SP Tambun at PT Pertamina EP Java Region Field Tambun 2013 has to be done because the presence of the hazards and risk potential that threats not only employee’s safety but also people’s safety around the pipeline Right of Way (ROW). This analytical descriptive research that uses semi-quantitative analytical method has a purpose to assess the existing risk score and risk level.
The focus of this research is to analyze all occupational accidents of lifting activities on land rig operations in PT ‘X’ using the Human Factor Analysis and Classifications System (HFACS) method in 2014 - 2018. The type of research methodology is qualitative research with a descriptive design. The final result shows that the unsafe act layer is the most ineffective layer that contributing to almost all occupational accident cases which is 45 of 49 total cases of occupational accidents. Error is the sub-layer of unsafe act which has the highest number of contributions to occupational accident cases with total 39 cases. On the other side, the organizational influences layer is the second layer that has high contribution to accident which is 26 of 49 total cases of occupational accidents. The organizational process is the sub-layer of organizational influences which contributing to 23 cases of occupational accident. The third layer which has contribution to accident is unsafe supervision. The unsafe supervision has contribution to accident which is 16 of 49 total cases of occupational accidents. Inadequate supervision and planned inappropriate operation are the sub-layer of inadequate supervision which contribute to the accident cases for 10 cases equally. The layer of preconditions for unsafe actions is the effective layer which has contribution to occupational accident cases which is 8 of 49 total cases of occupational accidents. Personnel factor is the sub-layer of preconditions for unsafe actions which contribute to 7 cases of occupational accidents. According to the result, researcher recommend that corrective action must be taken at each layer of HFACS as the safety protection system, both latent failures and active failures with the emphasis on improvement, which start from the organizational influences layer, followed by the unsafe supervisions layer, and then unsafe actions layer, while the improvement on the layer of precondition for unsafe actions becomes the last improvement. Improvement to organizational influences layer, unsafe act layer, and unsafe supervisions layer will have a positive influence on the layer of precondition for unsafe actions.

Work shift which is applied by the company can made a good or bad effect to the employee. One of the bad affect is a presence of fatigue symptoms. Study work shift against the symptoms of fatigue from % CVL are 56 respondents (100%) are not suffering from fatigue, 22 respondents (39,3%) from night shift workers are also not suffering from fatigue, 33 respondents (58,9%) are necessary to repairs and 1 respondents (1,8%) is working in a short time. On a morning work shift day 7th there is an increase 42,9% in a Hypertension Grade I, and on a night work shift day 7th there is an increase 46,5% on a Hypertension Grade II. The level of subjective fatigue on a morning shift there is 49 respondents (87,5%) are not suffering from fatigue and 7 respondents (12,5%) in a light fatigue. On a night shift there are 7 respondents (12,5%) not suffering from fatigue, 29 respondents (51,8%) in a light fatigue and 20 respondents (35,7%) in a mild fatigue. The level of sleepiness there are 56 respondents morning work shift (100%) in a normal level, on a night shift there are 32 respondents (57,1%) in a excessive sleepiness category and 24 respondents (42,9%) in a normal level.
