ABSTRAK Nama : Alifianti Lestari Program Studi : Kajian Administrasi Rumah Sakit Judul : EVALUASI PENGEMBANGAN PUSKESMAS KECAMATAN MENJADI RSUD KELAS D DENGAN STUDI KASUS DI 4 RSUD JAKARTA PUSAT TAHUN 2017 Penelitian ini dilakukan dengan pendekatan kualitatif yang secara umum bertujuan untuk mendapatkan hasil evaluasi pengembangan Puskesmas Kecamatan menjadi Rumah Sakit Kelas D di Jakarta Pusat tahun 2017. Penelitian dilakukan selama dua bulan ( April – Mei 2017 ) dengan mengambil lokasi di 4 RSUD Jakarta Pusat yaitu : RSUD Kemayoran, RSUD Cempaka Putih, RSUD Johar Baru dan RSUD Sawah Besar. Pengumpulan data melalui data primer berupa wawancara mendalam, yang dilengkapi dengan telaah dokumen dan observasi lapangan, kemudian data sekunder yang berasal dari 4 RSUD di Jakarta Pusat. Hasil penelitian menunjukkan banyak penyebab yang sangat mempengaruhi layanan kesehatan yang dberikan di 4 RSUD Jakarta Pusat. Seperti ketersediaan Dokter Spesialis yang tidak mudah mau bekerjasama dengan RSUD Kelas D di Jakarta Pusat. Salah satunya dikarenakan belum adanya kesepakatan pendapatan dokter spesialis. Selain itu kelengkapan sarana prasarana juga berpengaruh dalam pemberian layanan kepada masyarakat, lokasi Rumah sakit yang kurang strategis berpengaruh juga terhadap pemasaran layanan, serta kebijakan pengembangan Rumah sakit dengan layanan unggulan sesuai kebutuhan masyarakat sekitarnya. Sehingga pada penelitian ini dapat disimpulkan bahwa 4 RSUD di Jakarta Pusat cukup berhasil menjadi RSUD Kelas D di tahun 2017 dengan terus memperbaiki dan melengkapi sumber daya manusia termasuk Dokter Spesialis dan melengkapi sarana prasarana kesehatan rumah sakit yang belum terpenuhi secara optimal. ABSTRACT Name : Alifianti Lestari Study Program : Magister of Hospital Administration Title : EVALUATION OF PUSKESMAS KECAMATAN DEVELOPMENT BECOMING CLASS D HOSPITAL WITH CASE STUDY IN 4 HOSPITALS IN CENTRAL JAKARTA 2017. This research was conducted with qualitative approach which generally aimed to get the evaluation result of development of Puskesmas Kecamatan into Class D Hospital in Central Jakarta in 2017. The research was conducted for two months (April - May 2017) by taking the location in 4 Central Jakarta Public Hospital namely: Kemayoran Hospital, Cempaka Putih Public Hospital, Johar Baru Hospital and RSUD Sawah Besar. Data collection through primary data is in-depth interview, completed with document review and field observation, then secondary data coming from 4 hospitals in Central Jakarta. The results showed many causes that greatly affect the health services provided in 4 Central Jakarta hospitals. As the availability of Specialist Doctors who are not easy to cooperate with RSUD Class D in Central Jakarta. One of them is because there is no specialist doctor's income agreement. In addition, the completeness of infrastructure facilities also affect the provision of services to the public, the location of hospitals that are less strategic also affect the marketing of services, as well as development policies of hospitals with excellent service according to the needs of the surrounding community. So in this study it can be concluded that 4 hospitals in Central Jakarta succeeded in becoming RSUD Class D in 2017 by continuously improving and completing human resources including Specialist Doctor and equipping hospital health infrastructure facilities that have not been fulfilled optimally. This thesis discusses the feasibility of the South Tangerang City General Hospital with the service standards, human resources, equipment and infrastructure currently owned by South Tangerang General Hospital that have exceeded the standards of Class C hospitals. Rapid development in South Tangerang City with the highest human development index in Banten Province, with a population growth of 3 , 04% per year, 3.75% economic growth, high per capita income and with a growing population, based on the 2016-2021 South Tangerang City RPJMD there will be the construction of two class C hospitals in South Tangerang City and South Tangerang General Hospital will become Referral hospital and quality improvement to Class B required a feasibility study. This research is a case study research with a quantitative quantitative approach with secondary data analysis methods conducted at South Tangerang General Hospital using secondary data for 3 years (2017-2019). Increasing Class Upgrading of South Tangerang General Hospital to become a Class B hospital using self-assessment based on Permenkes No. 3/2020 and using a SWOT analysis on internal and external factors. From the results of the analysis of the fulfillment of service standards, human resources, buildings and infrastructure, and equipment, it has met the standards of a class B public hospital, only the fulfillment of the minimum standard for class B hospital beds has not been fulfilled, based on the situation of internal and external factors using SWOT analysis to get a value EFE 3.54 and IFE 3.21, are in cell I which supports the growth and upgrading of the South Tangerang City Hospital class into a Class B hospital. The policy foundation for sports medicine services in hospitals in Indonesia is still general in nature, so guidelines are needed. This study aims to develop guidelines of sports medicine services in class B general hospitals in Indonesia. This research is a qualitative study in 4 hospital locations, which took place from September to December 2020 with triangulation techniques and the 2-stage Delphi method, then analyzed. Currently, at the research location there are still variations in services, starting from the flow, workforce, infrastructure, equipment, and service delivery processes. Likewise in the Delphi method where there were 59 topics that failed to reach a consensus so that it needed to be analyzed and sought reinforcement from other sources. The recommendations for draft guidelines in ideal conditions (gold standard), namely services are carried out in a team led by sports medicine specialists and members consisting of managers, clinicians and sports science personnel with multidisciplinary and interdisciplinary concepts; gold standard infrastructure and equipment such as CT-Scan, DXA Scan, Echocardiography, and CPET need to be fully provided in one area; optimization of service flow; promotive efforts are given to all patients, trainers, and health workers; MCU involving the sports medicine unit from start to finish; doing exercise is medicine program according to FITT; supervision against doping; specific diagnostic coding; as well as athletic patients who need to be handled collaboratively from the start to return to sports, while non-athletic patients, transfer of patient care from a physical medicine specialist and rehabilitation to a sports medicine specialist is carried out after the patient is free of injury. This recommendation is expected to be the beginning of the preparation of an academic paper and then set as a policy to realize integrated services, one stop services, athlete /patient centered-care, and plenary.
The JKN program was launched in 2014 to provide wider access to health services for Indonesian citizens and even foreigners. The growth rate of JKN participants is increasing, from the lower classes of society as well as the rich people who. In the higher-class community, there is a tendency to demand extra services with upgrading class. There are factors that influence the patient's decision. The purpose of this study was to determine the relationship between the characteristics of JKN patients and the decision to upgrading class. This study uses a retrospective study design and a cross-sectional method, using secondary data from the medical records of patients and data on claims for JKN patients from January 1 to December 31, 2019. 171 samples studied, the patients who decide to upgrade to the VIP class mostly are female, JKN Workers participant segment, most of them make the decision based on the patient's wishes even though the inpatient room is available, and for non-surgical treatment. There is a significant relationship between gender, education and the availability of the rooms on the decision of JKN patients to upgrade the class. JKN participant segmentation and treatment measures did not have a significant relationship
