Abstrak:
Skripsi ini membahas kegiatan monitoring dan evaluasi program Therapeutic Feeding Centre (TFC) pada pasien pasca rawat inap. Tujuan dari penelitian ini untuk menganalisis kegiatan monitoring dan evaluasi program TFC pada balita pasca rawat inap. Penelitian ini merupakan penelitian kualitatif. Hasil penelitian menemukan penyebab orangtua tidak melakukan kegiatan monitoring dan evaluasi pasca rawat secara lengkap pada anaknya disebabkan oleh faktor keluarga, sosial ekonomi, jarak yang jauh dari rumah ke Puskesmas, dan faktor lain. Penelitian ini menunjukkan sebesar 80% program berpotensi mengalami kegagalan karena Puskesmas tidak berhasil mencapai target yang ditetapkan pada kegiatan monitoring dan evaluasi pasca rawat Kata Kunci: Evaluasi, Monitoring, Program This research explained about monitoring and evaluation of Therapeutic Feeding Centre (TFC) program on Post-hospitalization Patients. The purpose of this study was to analyze the activities of monitoring and evaluation of programs on Posthospitalization Patients. This research is descriptive survey with qualitative approach. The result of the research found cause of the parents does not complete the monitoring and evaluation activities on their post- hospitalization child due to family factors, socio-economic, a long distances from home to health center, and other factors. Abstrak:
Skripsi ini membahas mengenai perancangan prototipe sistem monitoring kinerja pegawai di BBPK Ciloto. Penelitian ini merupakan penelitian kualitatif dengan metode pengembangan sistem Iteratif dan Incremental sampai tahap prototipe halaman antar muka (interface). Berdasarkan hasil pengumpulan data yang dilakukan dengan metode wawancara kepada responden di BBPK Ciloto, ditemukan permasalahan dimana sistem monitoring kinerja pegawai yang berjalan masih manual mengakibatkan proses penilaian kinerja pegawai belum berdasarkan data yang terukur. Hal ini yang menjadi peluang peneliti untuk merancang sebuah prototipe sistem monitoring kinerja pegawai berbasis web dengan harapan proses monitoring hasil kerja pegawai dapat dilakukan secara real- time oleh atasan. Berdasarkan hal tersebut, dirancang sebuah prototipe sistem monitoring kinerja pegawai berbasis web.

Sistem angka kredit dosen Poltekkes Kemenkes yang berjalan saat ini belum dapat menjawab solusi permasalahan tentang monitoring dan pengusulan angka kredit dosen Poltekkes Kemenkes. Penelitian ini bertujuan untuk mengembangkan sistem informasi monitoring angka kredit dosen Poltekkes Kemenkes untuk mengetahui sejak dini permasalahan pemenuhan angka kredit dosen maupun yang telah memenuhi angka kreditnya. Pengembangan sistem informasi monitoring angka kredit dosen Poltekkes Kemenkes mengikuti alur siklus hidup pengembangan sistem (SDLC), yaitu perencanaan,analisis, desain dan implementasi. Metodologi yang digunakan adalah Rapid Application Development (RAD) dengan pendekatan prototyping. Penelitian ini menghasilkan proptotype sistem informasi monitoring angka kredit dosen Poltekkes Kemenkes dengan berbasis web. Sistem ini mempermudah dosen Poltekkes dalam memantau perolehan angka kredit dan usulan penetapan angka kredit dosen. Implementasi sistem dapat dilakukan di Badan PPSDM Kesehatan selaku pembina jabatan fungsional dosen berupa dukungan dana, infrastruktur dan SDM yang telah tersedia. Namun demikian diperlukan komitmen dari dosen selaku pengguna sistem untuk menggunakan sistem ini secara berkelanjutan dan tentunya kebijakan dari Badan PPSDM Kesehatan untuk membuat regulasi atau mandatory terkait sistem ini. Disamping itu pula perlu adanya sosialisasi ke Poltekkes Kemenkes khususnya dosen yang berinteraksi langsung ke sistem. Kata kunci: Sistem Informasi, Monitoring, Dosen

Suatu rancangan sistem informasi pelaporan monitoring dan evaluasi yangterkomputerisasi diciptakan untuk menggantikan sistem lama yang masih manual. Metodelogi menggunakan tahapan dalam metode System Development Life Cycle (SDLC) yang dimulai dari tahap analisis,perancangan database, perancangan interface hingga implementasi. Studiini dibatasi hanya sampai pada tahap desain sistem. Hasil pengembangansistem monitoring dan evaluasi pelaksanaan kegiatan dan realisasi anggaran kesehatan di Dinas Kesehatan Provinsi Bengkulu memudahkan penggunauntuk mendapat data alokasi anggaran, realisasi, capaian kegiatan , dan sisaanggaran. Abstrak:
Puskesmas dan Klinik yang merupakan gate keeper dalam memberikan pelayanan klinis kepada masyarakat harus dapat menyediakan pelayanan klinis tingkat pertama yang aman dan bermutu. Dengan kata lain, Puskesmas dan Klinik merupakan ujung tombak dalam memberikan pelayanan kesehatan kepada masyarakat. Prinsip penyelenggaraan Puskesmas yaitu pertenggungjawaban kesehatan di daerah kerjanya membuat Puskesmas memiliki kewenangan untuk mengkoordinir dan membina klinik yang berada di wilayah kerjanya. Namun, di Kota Bandung, pelaksanaan pengawasan dan pembinaan klinik oleh Puskesmas saat ini tidak seragam dikarenakan tidak adanya pedoman pelaksanaanya di lapangan dan di tingkat Dinas Kesehatan pengelolaannya belum berjalan. Dengan menerapkan metode System Development Life Cycle (SDLC) Prototyping, penelitian ini bertujuan membangun prototype pengawasan dan pembinaan klinik oleh Puskesmas. Tahapan penting yang dilalui adalah menyusun format standar pengawasan, menetapkan standar klinik yang sesuai dengan pengawasan dan pembinaan klinik oleh Puskesmas, dan menyusun informasi standar output. Terdapat 73 parameter yang terbagi ke 10 jenis kategori pengawasan dan 6 jenis keluaran yang dihasilakan oleh sistem ini yang salah satunya adalah klinik yang sesuai dengan standar pengawasan dan pembinaan Puskesmas. Teknologi internet, berupa prototype berbasis Web, yang memiliki keunggulan memangkas jarak dan waktu dapat menjadi solusi bersama. Tujuannya, selain keunggulan diatas, situs Web mampu berkomunikasi, melakukan jasa, mengumpulkan informasi dari penguna dan berbagi basis data dengan pihak-pihak yang telah ditentukan sebelumnya. Kebijakan yang mendukung pengembangan sistem informasi ini dapat dilakukan pada perumusan pedoman pelaksanaan kegiatan pengawasan dan pembinaan klinik oleh Puskesmas dengan mengundang lebih banyak pihak seperti oraganisasi profesi dan ASKLIN. Puskesmas and Clinics, the gatekeepers of social health services, as spearheads in providing first communal health services, always have to be on the top of their roles, giving the community their basic health services. Puskesmas as a senior partner, has the authority to nurture, coordinate, and to monitor its subordinate Clinics in order to implement their portion of health responsibility roles in the community. However, in the City of Bandung, the implementation of monitoring and supervising clinic by Puskesmas is not in the same shape over one another due unavaliable of implementation guidance in the field. The management of this activity at The District Health Office level has not been enabled yet. However, in the city of Bandung, the implementations of Puskesmas and Clinics monitoring- supervising activities differ one from another, due to the absence of a thorough implementation guideline that can act as a Puskesmas-Clinic uniformed. Meanwhile, at the District Health Office level, such guidance or system are also yet to be invented, controlled nor managed systematically. This research objective is to build a monitoring sytem prototype in which to help Puskesmas effectively performs one of its functions and intended roles in the community, which are to monitor and to supervise its subordinate Clinics. The system suitable to perform in this environment is called System Development Life Cycle (SDLC). Important steps are arrange the standard format of supervision, establishing clinic standards monitoring and supervision of clinic by the Puskesmas, and the preparation of standard output information. The important steps are therefore broke down into three main stages. The first stage is to arrange the standard form for supervision, to be followed by establishing clinical standard for monitoring and supervising Clinics by the Puskesmas, and the third stage is to arrange the information standard output. The SLDC is going to generate seventy three parameters, which are then divided into ten different supervisory catagories and six different type of outputs. One of the outputs is going to produce a Clinic that complies with Monitoring and Supervision Standards of Puskesmas. Web- based data, supported by lightning fast internet connection that shaves time and distance, can be offered as solution to become the backbone of SLDC. Furthermore, web-based technology is projected to have the ability to communicate, to provide services, to collect information from users, and even to be able to share data to many different type of pre-selected individuals or institutions.. The governing policies which will support the development of this information system can be proposed and formulated by Puskesmas under the Clinical Supervision and Monitoring Guidelines, with the help of other professional associates such as from ASKLIN or other professional institutions. Tingginya angka kematian pada pasien kanker serviks diakibatkan sebagian besar pasiendatang melakukan pengobatan sudah pada stadium lanjut. Kanker jika ditemukan distadium awal lebih dari setengahnya dapat dicegah bahkan dapat disembuhkan. Programdeteksi dini kanker serviks salah satunya dapat dilakukan melalui tes IVA dan apabiladitemukan lesi pra kanker serviks diikuti dengan pengobatan krioterapi. Masih ditemukankejadian losing follow-up pada pasien dengan hasil IVA Positif, masih terdapat wanitatidak melakukan krioterapi dikarenakan takut akan tindakan krioterapi. Dibutuhkansebuah sistem yang dapat melakukan monitoring secara otomatis untuk mengurangikejadian losing follow up. Pengembangan sistem mengunakan System Development LifeCycle (SDLC) dengan metode prototyping. Pengembangan sistem ini melibatkanpenguna dalam melakukan analisa kebutuhan, sehingga sistem yang dikembangkansesuai dengan kebutuhan pengguna, sistem dapat diakses baik dari sisi tenaga kesehatanatau pun pasien, inputan hasil pemeriksaan dilakukan oleh tenaga kesehatan, yangkemudian secara otomatis mengirimkan sms reminder kepada pasien dengan hasil IVAPositif yang telah dijadwalkan melakukan tindakan krioterapi, sistem juga memberikansms reminder kepada petugas kesehatan untuk menghubungi dan mengintervensi pasienyang tidak hadir melakukan krioterapi sampai dengan batas waktu tertentu, untuk sisipasien, pasien dapat melihat hasil skrining terdapat juga artikel yang berkaitan denganpendidikan mengenai kanker serviks, menu edukasi ini dapat pengetahuan pasienmengenai kanker serviks. Pengembangan sistem informasi ini dapat meningkatkankemampuan monitoring dengan adanya fitur sms reminder sehingga dapat mengurangikejadian losing follow up pada pasien dengan lesi pra kanker serviks, disamping ituPasien dimudahkan dalam mendapat informasi terkait hasil skrining kanker serviks danpengobatan lanjutan, sistem ini juga dapat meningkatkan pengetahuan pasien denganmenu-menu edukasi yang terdapat pada aplikasi ini.Kata kunci:Kanker Serviks, Sistem Informasi, Monitoring, SMS Reminder.

Pencemaran udara yang mengandung Particulate Matter (PM) baik dalam jangka panjang maupun pendek telah diketahui dapat menyebabkan kematian dan efek kesehatan terutama pada jantung serta paru-paru. Polisi Satgatur Polda Metro Jaya merupakan salah satu pekerja yang terpajan akan pencemaran udara tersebut. Maka dari itu penulis melakukan penelitian pada Polisi Satgatur Polda Metro Jaya untuk mengetahui gambaran konsentrasi pajanan PM 10 dan 2,5 yang berasal dari pencemaran udara terutama hasil pembakaran kendaraan bermotor di Pos Polisi Harmoni, Bundaran HI dan Bundaran Senayan. Metode yang digunakan dalam penelitian ini adalah dengan menggunakan analisis Gravimetri untuk mengetahui konsentrasi dari partikulat. Hasil yang diperoleh pada penelitian ini yaitu konsentrasi PM 2,5 dan 10 terbesar di Bundaran Senayan pada shift 1 disaat hari kerja dan jika dibandingkan dengan standar ACGIH, NIOSH dan OSHA belum melebihi nilai ambang batas. Pendokumentasian rekam medis harus dibuat selengkap dan seakurat mungkin guna untuk meningkatkan kualitas pencatatan medis yang menjadi bagian yang sangat penting dalam pengelolaan manajemen informasi kesehatan. Rekam medis lengkap dapat menjadi cerminan baik buruknya pelayanan pelayanan serta rekam medis merupakan sumber utama untuk bahan pengambilan keputusan rencana pengobatan, bahan pelaporan dan riset, dan dokumen penting dalam proses pembiayaan klaim JKN. Monitoring dan evaluasi kelengkapan pengisian secara berkala dan terintegrasi diperlukan sebagai intervensi dalam peningkatan kualitas pencatatan rekam medis. Penelitian ini bertujuan untuk merancang prototype sistem informasi yang berguna bagi fasilitas pelayanan kesehatan untuk melakukan monitoring dan evaluasi kelengkapan rekam medis. Hasil evaluasi penerimaan pengguna pada kelompok uji coba dengan mengadopsi Technology Acceptance Model (TAM) menyatakan bahwa 80% responden menyatakan sangat setuju bahwa sistem informasi mudah digunakan, 83% menyatakan sangat setuju bahwa sistem memiliki manfaat untuk menunjang kinerja dan hasil penilaian sikap penggguna 83% responden menyatakan sangat setuju untuk menggunakan sistem informasi. Saran peneliti kepada pemangku kebijakan di rumah sakit agar dapat diimplementasikan sistem informasi yang telah dikembangkan yang berguna sebagai alat bantu dalam kegiatan monitoring dan evaluasi untuk peningkatan kelengkapan dan kualitas pengisian rekam medis di institusi pelayanan kesehatan.
Medical records are a collection of data containing clinical notes, demographic data, and the treatment history provided to patients. Documentation of medical records must be made as complete and accurate as possible to improve the quality of medical record-keeping, which is a very important part of health information management. Complete medical records can reflect the quality of services provided and are the main source for decision-making in treatment planning, reporting, research, and important documents in the JKN claim financing process. Periodic and integrated monitoring and evaluation of the completeness of medical record entries are necessary interventions to improve the quality of medical record-keeping. This research aims to design a prototype information system useful for healthcare facilities to monitor and evaluate the completeness of medical records. The development of the information system adopts the System Development Life Cycle (SDLC) model, resulting in system requirements data, feasibility analysis that this system can be developed through a feasibility study approach, and producing logical designs for database architecture and web-based application prototypes. User acceptance evaluation results in the test group, adopting the Technology Acceptance Model (TAM), indicate that 80% of respondents strongly agree that the information system is easy to use, 83% strongly agree that the system is beneficial in supporting performance, and 83% of respondents strongly agree to use the information system. The researchers suggest that policymakers in hospitals implement the developed information system as a tool in monitoring and evaluation activities to improve the completeness and quality of medical record entries in healthcare institutions.
