Patient safety still is a global issue throughout the world with the high incidence ofhospital patient safety. One way to control patient safety incident rate is through thedevelopment of a reporting system. This study discusses the description of the reportingof hospital patient safety incidents in Indonesia with a literature review design. This studyaimed to get an information about the description of hospital patient safety incidentreports reviewed in Indonesia in terms of the availability of policies and methods usedalso the factors that hinder the reporting of patient safety incidents. This study uses aliterature review design with a database of PubMed, Science Direct, CINAHL, GoogleScholar and Garuda Research and Technology. The search results show that mosthospitals in Indonesia already have a patient safety incident reporting policy and use theoffline reporting method with a form. Broadly speaking, the hospital's IKP reportingprocedures are in accordance with the National Patient Safety Guidelines. But in practicethere are still many that are not in accordance with the reporting flow that should be andstill encounter many obstacles, such as fear of reporting, lack of socialization, lack ofknowledge and motivation, complicated reporting systems, low patient safety culture, noreward, and feedback which is not optimal yet. Results showed, it was found that theoutput from incident reporting such as timeliness and completeness are not yet fullyachieved.Key words:Patient safety incident report, hospital, Indonesia, literature review.
Penelitian ini bertujuan untuk mengetahui Kinerja Pegawai Dinas Kesehatan Kota Palangka Raya. Jenis penelitian ini adalah survei cross-sectional dengan menggunakan kuesioner penilaian kinerja berdasarkan metoda penilaian sendiri dan metoda 360° yang dimodifikasi. Populasi dan sampel adalah semua Pegawai Negeri Sipil Dinas Kesehatan Kota Palangka Raya sebanyak 69 orang. Dari 10 unsur penilaian kinerja, ada 6 (enam) unsur yang telah terbukti tidak memiliki perbedaan yang bermakna yaitu pengetahuan terhadap pekerjaan, kuantitas kerja, kualitas kerja, kreativitas, kemauan belajar dan inisiatif, dan komunikasi, sementara 4 (empat) unsur lainnya seperti kerjasama, integritas, disiplin kerja, dan kepemimpinan memiliki perbedaan yang bermakna. Berdasarkan hasil penelitian ini, diperlukan perbaikan manajemen dan peningkatan efektivitas kepemimpinan di lingkungan Dinas Kesehatan Kota Palangka Raya.
This study aims to determine the Performance of Employee of Palangka Raya Municipality Health Office. This type of research is a cross-sectional survey using modified questionnaires of performance asssessment based on self assessment and 360° methods. Population and sample are all Civil Servants of Palangka Raya Municipality Health Office as many as 69 persons. Of the 10 elements of performance assessment, there are 6 (six) elements that have been proved no significant difference such as knowledge of work, quantity of work, quality of work, creativity, willingness to learn and initiative, and communication, while 4 (four) other elements like cooperation, integrity, work discipline, and leadership have significant differences. Based on the results of this study, improved management and increased effectiveness of environmental leadership are needed in Palangka Raya Municipality Health Office.
ABSTRAK Tesis ini membahas tentang kelayakan rujukan oleh bidan Puskesmas Poned Kota di RSUD Pirngadi Medan Tahun 2012. Penelitian ini adalah penelitian kuantitatif dengan desain cross sectional. Sampel pada penelitian ini adalah seluruh kasus rujukan yang dirujuk ke RSUD Pirngadi berjumlah 136 kasus rujukan, dan dilengkapi dengan informasi dari 17 orang bidan di 6 Puskesmas Poned. Data primer diambil dengan wawancara menggunakan kuesioner, daftar ceklist dan data sekunder menggunakan data rekam medis RSUD Pirngadi Medan. Ada 75,7% kasus rujukan persalinan yang layak dirujuk dan 24,3% kasus rujukan persalinan yang tidak layak dirujuk. ABSTRACT This study analyzed the referral feasibility by Poned Health Centres midwives in Pirngadi Medan Hospital in 2012. This is a quantitative research using cross sectional design. Sample in this study are all referral cases referred to Pirngadi Hospital 136 cases and supplemented by information from 17 midwives in 6 Poned Health Centre. Data used a questionnaire, checklist, and medical record. There were 75,7% of the cases referred to decent referral and 24,3% of cases referral is not feasible referenced.
This study uses the Narrative Review method concerning the analysis of theLean Thinking concept towards improving the waiting time for patient services in theOutpatient Hospital in Indonesia. The purpose of this study was to describe theimplementation of the Lean Thinking concept in overcoming the waiting time for patientservices in the Hospital Outpatient Installation. In order to obtain test-worthyliterature, researchers used the PRISMA guidelines. The researcher obtained 12literatures using qualitative and quantitative research methods. The results of the studyconclude that the Lean concept can prove that the outpatient service process ofhospitals in Indonesia is still not Lean. There are 7 literatures that show that Lean iseffective in reducing the waiting time of outpatient services at the hospital as evidencedby adopting Lean's main principles, namely Standardize work and Heijunka. In order toachieve the Lean condition, the Hospital must align the implementation of the 5 Leanprinciples (customer value, value stream, flow, pull, perfection) with organizationalculture, mindset, Lean method, cultivate the 5 S, by involving all parties in the hospitalto solve problems consistently and continuously.Keywords:Outpatient Waiting Time; Lean Hospital.
