Long waiting times in the outpatient care would hamper the service, have an impacton queues and will end up with an inefficient service. This qualitative research,using the lean method, observed the time spent by the outpatients while in hospitalwith the aim to improve the efficiency of outpatient services at the Sumber WarasHospital Ciwaringin with lean method.The results showed that 90% of the service time is non-value added activities andonly 10% is counted as value added activities. Simulation is applied and has leadto improvement in the process of outpatient services by decreasing the non valueadded activities to 70,59% and increasing the value added activities to 29,41%

Hasil penelitian menunjukkan wakturata-rata yang diperlukan untuk pasien metode pembayaran BPJS sebesar 153,5menit, Cash sebesar 127 menit , dan asuransi sebesar 264 menit. Total persentasenon value added activity pada proses pemulangan pasien dengan metodepembayaran BPJS sebesar 76,4%, cash sebesar 72,4%, dan asuransi sebesar84,1%.
Usulan perbaikan dengan metode Lean dapat menurunka persentase nonvalue added activity menjadi sebesar 56,6% untuk pasien BPJS, 46,6% untukpasien cash, dan 56,8% untuk pasien asuransi.

Delays in the process of discharge related to the availability of the number of bedsfor patients who are going to be hospitalized. This resulted inefficiency ofservices. Lean method is a method that is expected to improve efficiency in theprocess of discharge process. A qualitative study using the principles of leanthinking to describe the process of discharge flow, calculate the lead time andcycle time, and analyze the waste that occurs.
The results show the average timerequired for patient used BPJS payment method is 153.5 minutes, cash is 127minutes, and insurance is 264 minutes. The total percentage of non-value addedactivity in the process of discharge process with BPJS payment method is 76.4%,cash is 72.4%, and insurance is 84.1%.
Suggestions for improvements with leanmethods can reduce the percentage of non-value added activity by 56.6% forBPJS patients, 46.6% for cash patients, and 56.8% for insured patients.

Penelitian ini bertujuan memperbaiki angka Kejadian Tidak Diharapkan (KTD) di ruang rawat biasa Instalasi Rawat Inap RS Anna Medika dengan metode Six Sigma. Penelitian ini menggunakan pendekatan kualitatif yang ditunjang dengan metode kuantitatif.
Hasil penelitian menyarankan pengembangan yang paling tepat adalah pengawasan langsung kepada petugas kesehatan dan penempatan sabun disertai label peringatan di tiap wastafel kamar perawatan, disertai upaya untuk meminimalisir biaya pelatihan berkala, evaluasi hasil pengembangan, modifikasi komponen biaya, dan pemberdayaan seluruh petugas untuk mendukung program tersebut.
This research aims to improve adverse event rate in regular care on inpatient service of Anna Medika Hospital using Six Sigma method. Qualitative approach with quantitative method support is used in this research.
The results suggest hospital to improve adverse event rate by observing of health workers hand higiene and procedures, and providing liquid soap with warning sign hand washing sink in every patient's room.

This study analyzes the waiting time of prescription services in Installation of Pharmacy Awal Bros Hospital, Bekasi. It is a qualitative and quantitative research, with sampel of 172 prescriptions, both concoction and non concontion drugs. This study found that the average waiting time of conconction drugs is 41 minutes, while the non concoction drugs is 22 minutes. The factors affecting those waitng time is insufficient of human resources, lack of drugs provision, inadequate correction of IT system lack of work experience, improper working space for doing concoction drugs.

Hospitals have an obligation to provide safe, high quality, anti-discriminatory and effective services. One of them is pharmacy services as an integral part of the hospital service system that is responsible for ensuring the availability of safe, high quality and efficacious drugs. KMK 129/Menkes/SK/II/2008 states that hospitals are required to meet the Standards of Minimum Services (SPM), one of which is the Waiting Time for Drug Services. RS Jantung Hasna Medika Cirebon serves 4000-5000 outpatient visits every month with 92% of them being BPJS Kesehatan patients. The average waiting time for concoction drugs was only achieved by 72% and non-concoction drugs achieved by 40%. The purpose of this study was to increase the speed of outpatient prescription services for BPJS Kesehatan patients at the pharmacy installation of the RS Jantung Hasna Medika Cirebon using Lean Hospital. This research method is operational research with a qualitative approach. Primary data sources taken through direct observation with time and motion study techniques, in-depth interviews and Focus Group Discussion (FGD). The results of the study found 2 types of waste, namely 99.31% waiting waste and 0.69% motion waste, after intervention in the form of optimizing e-prescription, 5S and visual management as well as continuous flow and process balancing there was a decrease in lead time from 01:24:47 to 00:25:30 or decreased by 59 minutes 17 seconds (69.93%). The conclusion of this study is that Lean Hospital is the right method or tool to increase the value to waste ratio by reducing wasting time and increasing additional value for patients. This research has not been maximized because the choice of lean hospital tools is very limited considering the time is short. The researcher's suggestion is to encourage the hospital management to use this research as the first step of continuous improvement by using other tools in another service units

One way to improve efficiency, improve service quality and improve patient safetyin the United States by using the concept of Lean Thinking is applied in hospitalsbecome Lean Hospital. This study analyzes the service flow in Outpatient ClinicSpecialists as the data for improvement in Hospital X which is a Class B PrivateHospital Education. Using the methodology of operational research studies,conducted in-depth observation and interviews show that the non-value addedactivities can be up to 80% and value added activity is only 20%. The data showsthat there has been a waste (waste) and the results of the analysis of the roots ofthe problem using Root Cause Analysis (RCA) demonstrates that there are severalfactors that cause inefficiencies in ambulatory care specialist clinic. the proposedimprovements to reduce waste is divided into three stages, including the short-term, medium-term and long-term is expected to improve outpatient care andpatient satisfaction.
