Emergency department (ED) is a part of hospital which giving advancedservices. In dr. Mohammad Hoesin (RSMH) Palembang hospital already own anemergency department based on SK director of the hospital whom establishes theorganizational structure, duties and responsibilities, vision and mission, and standardoperating procedures emergency services. ED RSMH Palembang is lead by aspecialist urology and assisted by two heads of the room. Standard service of ED hasimplementing service standards according to accreditation standards KARS 2012.ED in the hospital arranged Indonesian health minister No.865/Menkes/SK/IX/2009 about ED standards. The head of health minister regulatesthe standardization of emergency services at the hospital, which managing standardorganizations, human resources, services, completeness infrastructure in ED. RSMHPalembang has been implemented specialist doctors duty on site in the ER sinceJanuary, 30th 2014 as a follow-up of the head of the Indonesian health minister. Eversince implemented a policy specialist on duty in the ER site still found thecompliance of the doctors are still not optimal and although the quality of service hasimproved in line with acreditation hospital KARS version 2012.This research aims to determine how the implementation of policy specialistsdoctors on site in the ER has been implemented in accordance with the expectedgoals in accordance with the head of health minister. Research done with qualitativemethod by performing in-depth interviews on informants. Informants interviewed areRSMH Palembang board of directors, chairman of the medical committee, chieffinancial officer, head of the ED room and specialist doctors. Assessment interviewresults are using logical framework policy implementation model George Edward IIIwith variable resources, communications, disposition and organizational structure.From the results of this study, the implementation of policy specialist doctorson site guard has not run well, due to the communication factor, disposition andorganizational structure has not been going well and much needed resource support.The given proposal is the addition of appropriate power and competence standards,the revised SOP, provision of communication media, improvement of facilities,improving the coordination and monitoring functions regularly, advocacy to the headof the Indonesian health minister.Keywords:Implementation, Emergency Department, George Edward III
Latar belakang Berbagai faktor diketahui memengaruhi kepuasan dan loyalitas pasien Instalasi Gawat Darurat (IGD) sebuah rumah sakit, mulai dari mutu layanan, harga hingga citra merek. Kepuasan dan loyalitas pasien IGD Rumah Sakit Eka Cibubur masih menjadi salah satu permasalahan dari keseluruhan pelayanan Rumah Sakit, yang berdampak pada kinerja operasionalnya berupa pertumbuhan negatif pada tahun 2023. Tujuan Tujuan utama penelitian ini adalah menganalisis dan menguji berbagai faktor yang memengaruhi kepuasan dan loyalitas pasien Instalasi Gawat Darurat (IGD) Rumah Sakit Eka Cibubur. Metode Merupakan studi kuantitatif pada 163 pasien yang menggunakan layanan IGD Rumah Sakit Eka Cibubur pada Desember 2023 hingga Januari 2024. Data diambil dengan menyebarkan questionnaire kepada pasien yang telah ditentukan setelah diujicobakan terlebih dahulu. Kemudian dilakukan analisis multivariat serta estimasi hubungan saling ketergantungan antara variabel laten secara bersamaan melalui uji Stuctural Equation Modeling (SEM). Hasil Kepuasan pasien dipengaruhi oleh mutu layanan (p = 0,034), harga (p = 0,000) dan citra merek (p = 0,015). Sedangkan loyalitas pasien hanya dipengaruhi oleh citra merek (p = 0,003) dan kepuasan pasien (p = 0,003). Selain itu, kepuasan pasien memediasi penuh hubungan antara harga (p = 0,024), namun tidak memediasi hubungan antara mutu layanan (p = 0,109) dan citra merek (p = 0,068) dengan loyalitas pasien. Kesimpulan Mutu layanan, harga dan citra merek yang dipersepsikan dengan baik oleh pasien IGD terbukti memengaruhi kepuasan akan layanannya. Dengan loyalitas akan penggunaan layanan berikutnya sangat ditentukan oleh kepuasannya, khususnya kepuasan akan harga yang terbukti sangat memengaruhi loyalitas pasien akan penggunaan layanan IGD selanjutnya. Background Various factors are known to influence patient satisfaction and loyalty in a hospital's Emergency Room (IGD), ranging from service quality, price to brand image. Satisfaction and loyalty of emergency room patients at Eka Cibubur Hospital is still one of the problems of the hospital's overall services, which has an impact on its operational performance in the form of negative growth in 2023. Objective The main objective of this research is to analyze and test various factors that influence patient satisfaction and loyalty at the Emergency Department (IGD) of Eka Cibubur Hospital. Methods This is a quantitative study on 163 patients who used the emergency room services at Eka Cibubur Hospital from December 2023 to January 2024. Data was collected by distributing questionnaires to patients who had been determined after being tested first. Then, multivariate analysis was carried out and estimation of the interdependence relationship between latent variables simultaneously through the Structural Equation Modeling (SEM) test. Results Patient satisfaction is influenced by service quality (p = 0.034), price (p = 0.000) and brand image (p = 0.015). Meanwhile, patient loyalty is only influenced by brand image (p = 0.003) and patient satisfaction (p = 0.003). In addition, patient satisfaction fully mediates the relationship between price (p = 0.024), but does not mediate the relationship between service quality (p = 0.109) and brand image (p = 0.068) and patient loyalty. Conclusion Service quality, price and brand image that are well perceived by emergency room patients have been proven to influence satisfaction with their services. Loyalty regarding subsequent use of services is largely determined by satisfaction, especially satisfaction with price which has been proven to greatly influence patient loyalty regarding subsequent use of ER services. The increasing number of patients visit in the Emergency Unit of Bali RoyalHospital is not followed by a change in the number of nurses. The purpose of thisstudy was to determine the workload of nurses and nurses needs in the EmergencyUnit of Bali Royal Hospital. This research was a descriptive quantitative research.Observations were made by the researcher to determine the time spent by nurses inserving patients by using the Time Motion Study. The results shown the workloadfor critically emergency patients at 63.4 minutes per day (1.1 hours per day),critically non emergency patients 1,585.5 minutes per day (26.4 hours per day), and2,187 minutes per day (36.5 hours per day) for non critical-but emergency patientsand the need for nurses in the Emergency Unit of Bali Royal Hospital was 17 people.Nurses needed additional power as much as 3 people.Keywords : emergency unit, a nurse, time motion study, workload, the need fornurses
Rumah Sakit Atma Jaya tidak mempunyai suatu bentuk penilaian kinerja yang komprehensif dan data Rumah Sakit Atma Jaya menunjukkan bahwa 75% pasien Inslalasi Rawat Inap berasal dari Instalasi Gawat Darurat, untuk menjadikan Instalasi Gawat Darurat sebagai andalan Rumah Sakit Atma Jaya maka Instalasi Gawat Darurat Rumah Sakit Atma Jaya perlu dievaluasi kinerjanya yang selanjutnya menjadi bahan masukan bagi Rumah Sakit Atma Jaya dengan memakai pendekatan balance scorecard maka kinerja Instalasi Gawat Darurat dapat dievaluasi dari komitmen dan kepuasan kerja sumber daya manusia di Instalasi Gawat Darurat, pelayanan pasien di Instalasi Gawat Darurat, kepuasan pasien di Instalasi Gawat Darurat dan kinerja keuangan Instalasi Gawat Darurat.Penelititan ini dilakukan secara deskriptif kualitatif dan kuantitatif dengan responden sebanyak 23 orang sumber daya manusia di Instalasi Gawat Darurat, 288 orang pasien yang datang berobat di Instalasi Gawat Darurat selama bulan Mel tahun 2002 yang dipilih secara random. Alat penelititan yang digunakan yaitu : wawancara mendalam, kuesioner, dan check list. Data yang dikumpulkan dianalisa secara deskriptif kualitatif dan kuantitatif.Hasil penelitian menunjukkan bahwa indikator sumber daya manusia di Instalasi Gawat Darurat belum baik yaitu mereka tidak puas dan tidak komitmen terhadap Instalasi Gawat Darurat. Akibat dari indikator sumber daya manusia yang belum baik tersebut mempengaruhi proses pelayanan pasien di Instalasi Gawat Darurat. Indikator proses pelayanan yang belum baik tersebut mempengaruhi proses kepuasan pasien Instalasi Gawat Darurat terutama terhadap waktu tunggu rawat dan pelayanan dokter. Pasien yang tidak puas terhadap pelayanan yang diterima di Instalasi Gawat Darurat memberi dampak kurang baik terhadap pemasukan keuangan Instalasi Gawat Darurat ke rumah sakit sehingga rumah sakit tidak mampu memberi subsidi sebesar yang diharapkan sumber daya manusia di Instalasi Gawat Darurat. Hendaknya Direktur Rumah Sakit Atma Jaya menggunakan hasil penelitian ini sebagai dasar penilaian kinerja selanjutnya dan meninjau ulang kebijakan yang ada untuk meningkatkan kinerja di rumah sakit.
Performance Evaluation of Emergency Department at Atma Jaya Hospital in May 2002 Atma Jaya hospital doesn't have the comprehensive performance evaluation. There are 75% patients of in patient department come from emergency department, therefore emergency department need to become Atma Jaya hospital priority for performance evaluation then it will become an input in Atma Jaya hospital strategic planning. By using balanced scorecard approach, the performance of emergency department can be evaluated through its commitment and the work satisfaction of human resource in the emergency department, the process of patient service in the emergency department, the satisfaction of the emergency department?s patient, and the performance of the financial of the emergency department.This research was carried out by qualitative and quantitative descriptive, by using 23 respondents? human resources of emergency department, 288 patients who came to be cared in the emergency department during May 2002. The research was done by in depth interview, questioner, and checklist. The collected data were analyzed by qualitative and quantitative descriptive.The result of the research shows that human resource indicator in emergency department is not so good i.e. they are not satisfied and they do not commit to emergency department. The effect of that not so good human resource indicator influences the patient?s service process to the patient in emergency department. That not so good service process indicator influences the satisfaction of the emergency department's patient. The unsatisfaction of emergency department's patients especially about waiting time of care and the doctor's services. The unsatisfied patients to the service in emergency department causes not so good effects to the emergency department's earning to the hospital, so the hospital can not give subsidy as much as subsidy hoped by the human resource of emergency department. Atma Jaya Hospital can uses this research to be hospital foundation for next becoming performance evaluation, and Board of Director must observe at a distance about Atma Jaya Hospital policy that uses to raise performance of emergency department.
Rumah sakit Pusat Angkatan Darat Gatot Soebroto merupakan rumah sakit rujukan tertinggi bagi Rumah Sakit TNI diseluruh penjuru nusantara, Salah satu pelayanan di Rumah Sakit Gatot Soebroto adalah Instalasi Gawat Darurat yang merupakan pintu gerbang terdepan pelayanan di Gatot Soebroto. Salah satu standart dan prosedur adalah pelaksanaan Triase di Instalasi Gawat Darurat yaitu menyeleksi dan menilai kegawat daruratan dari setiap pasien untuk segera dapat diberikan pertolongan sesuai dengan tingkat kegawatan dan kedaruratan kemudian dicatat hasil pemeriksaan pada lembaran gawat darurat. Dari pengamatan awal membeikan hasil yang tidak sesuai dengan standar dan prosedur Triase yaitu tidak adanya keberadaan petugas Triase yaitu dokter dan perawat di ruangan Triase, ketidaktepatan kehadiran jam pelayanan petugas Triase, ketidak lengkapan pengisian daftar registrasi pasien baru datang ke Instalasi gawat darurat. Dalam penelitian ini dilakukan pengamatan mengenai faktor individu, psikologis dan organisasi terhadap kepatuhan petugas Triase terhadap standar dan prosedur Triase di IGD. Setelah dilakukan uji regresi linier sederhana dan uji t independent maka dapat disimpulkan bahwa umur perawat, pengetahuan dokter dan perawat, pelatihan dokter dan perawat, komitmen pimpinan dokter dan kecukupan sarana dari segi perawat ada hubungan /perbedaan dengan kepatuhan petugas dalam melaksanakan standar dan prosedur Triase. Untuk menjaga dan meningkatkan mutu pelayanan yang ada di Triase perlu diupayakan petugas Triase selalu berada di area Triase karena Triase merupakan pintu utama untuk penanganan pasien di IGD.Studi ini juga menganjurkan agar komitmen pimpinan terhadap petugas Triase lebih ditingkatkan sehinggan dapat meningkatkan motivasi petugad Traise untuk patuh terhadap standard an prosedur Triase dengan harapan pelayanan yang diberian sesuai dengan standar yang ada dan dapat dipertanggung jawabkan.

One is the implementation of standards and procedures in the unit Emergency Department Triage of selecting and assessing kegawat daruratan from each patient for immediate relief may be granted in accordance with the level of severity and emergency then recorded the results of the examination in the emergency sheet. From initial observations results inconsistent with the standards and procedures is not the existence of Triage Triage officers are doctors and nurses in the room Triage, inaccuracies Triage officer attendance at services, lack of charging lengkapan registration list of new patients coming to the emergency installation. In this study observations on individual factors, psychological and organizations for compliance officers to standards and procedures Triage Triage in the emergency room. After a simple linear regression test and independent t tests it can be concluded that the age of nurses, doctors and nurses the knowledge, training doctors and nurses, physician leadership commitment and the adequacy of existing nursing facilities in terms of the relationship / differences with the compliance officer in implementing standards and procedures Triage.
