Syahrizal Syarif, MPH, Ph.D Stroke Associated Pneumonia (SAP) adalah komplikasi stroke yang paling sering terjadi dan memiliki angka mortalitas yang tinggi. Faktor-faktor yang mempengaruhi mortalitas pasien SAP belum sepenuhnya diselidiki. Pengetahuan tentang faktor-faktor yang mempengaruhi mortalitas dapat membantu pengambilan keputusan klinis untuk tatalaksana pasien. Penelitian ini bertujuan untuk mengetahui faktor-faktor yang mempengaruhi mortalitas pasien SAP yang dirawat di Stroke Care Unit (SCU) Rumah Sakit Pusat Otak Nasional (RSPON) tahun 2016-2018. Desain penelitian ini adalah penelitian kohort retrospektif, pada 268 pasien di SCU RSPON yang didiagnosis SAP selama tahun 2016-2018. Variabel yang berhubungan bermakna dengan mortalitas pada pasien SAP adalah kesesuaian LOS dengan CP RSPON, didapatkan risiko 0,262 (95% CI : 0,138 – 0,501) yang berarti pasien dengan Length of Stay (LOS) tidak sesuai dengan Clinical Pathway (CP) RSPON memiliki hubungan protektif terhadap mortalitas pasien SAP yaitu sebesar 0,26 kali. Dengan menggunakan model prediktor, dapat dihitung probabilitas terjadinya mortalitas pada pasien SAP. Kata kunci: kohort retrospektif; mortalitas; Stroke Associated Pneumonia

Syahrizal Syarif, MPH, Ph.D Stroke Associated Pneumonia (SAP) is the most common complication after stroke and has a high mortality rate. Determining factors for SAP mortality have not been fully investigated. Knowledge of its determining factors can help clinical decision making for patient management. The aim of this study was to determine the mortality factors of SAP who were treated in the Stroke Care Unit (SCU) National Brain Centre (NBC) Hospital. This study used retrospective cohort design, involving 268 subjects SAP obtained at SCU NBC who were diagnosed with SAP during 2016-2018. Variable that significantly related is suitability of Length of Stay (LOS) with Clinical Pathway (CP) NBC obtained the risk of 0.262 (95%CI : 0,138-0,501) which means has protective association with mortality. Stroke is the third disease which often cause death in Indonesia. Prevalence of stroke in Indonesia from Riskesdas 2007 is 8,3 per 1000 person. Prevalence of stroke in Jakarta is still higher then national prevalence that is 12,5 per 1000 person and cause a lot of problems both in terms of social and economic. This research aims to identify different risk factors of ischemic and hemorragic stroke in inpatient of stroke at National Cardiovascular Center Harapan Kita 2012. The study was conducted by analyzing secondary data from patient medical record by using cross sectional study. Results showed that there were patients with ischemic stroke (10,4%) and hemorrhagic (89,6) with a characteristic age ≥62 years (51,5%), male (62%), low education (17,8%), smoking behaviour (37,4%), hypertension (81,6%) and diabetes (50,3%). Based on bivariate analysis, result shows that there is no statistical difference between risk factors and incidence of stroke, but the difference proportion of risk factors in ischemic stroke always greater than hemorrhagic stroke.
Coronavirus Disease 2019 (COVID-19) is an infectious disease caused by the Severe Acute Respiratory Syndrome Coronavirus 2 (SARS-CoV-2). In 2022, COVID-19 was the leading cause of inpatient admissions and deaths at Tarakan Regional General Hospital (RSUD Tarakan). This highlights the importance of investigating factors related to mortality of COVID-19 inpatient at RSUD Tarakan in 2022. This study uses a retrospective cohort design with secondary data from the hospital's Electronic Health Records (EHR). The variables studied included patient’s characteristics, comorbid, complications, and the management of COVID-19 cases. A total of 550 samples met the criteria for analysis. Multivariate analysis using cox regression with time constant identified several risk factors for COVID-19-related deaths at RSUD Tarakan in 2022, including severity (aRR 2.95; 95%CI 1.61-5.39), chronic kidney disease (aRR 3.19; 95%CI 1.62-6.31), and the duration of invasive mechanical ventilation for <96 hours (aRR 3.50; 95%CI 1.97-6.22) and ≥96 hours (aRR 2.44; 95%CI 1.24-4.80). Meanwhile, protective factors include the use of favipiravir (aRR 0.33; 95%CI 0.16-0.70) and remdesivir (aRR 0.69; 95%CI 0.38-1.26). Based on these findings, the researchers recommend that hospital improve clinical pathways and standard operating procedures that take into account the risk and protective factors for COVID-19 mortality
The increase in the number of cases of COVID-19 and its spread in various countries occurred quite quickly and in a short time. As of May 4, 2021, the total global confirmed cases of COVID-19 were 152,534,452 with 3,198,528 deaths. The Indonesian government has reported 1,682,004 people with confirmed COVID-19, the highest in Southeast Asia. A total of 45,949 COVID-19-related deaths were reported, with a CFR of 2.7%. Deaths from COVID-19 in Indonesia are ranked 2nd in Asia and 17th in the world. A case-control study was conducted using medical records of COVID-19 patients at Haji Adam Malik General Hospital in Medan for the period March 2020-December 2020. This study describes the characteristics and identifies factors associated with death in COVID-19 patients. The results of the study through multivariate logistic regression analysis showed that there was an increased risk of death at age 60 years (OR = 5,495, 95% CI: 2,398-12,591), fever (OR = 4,441, 95% CI: 1,401-14,077), shortness of breath. breath (OR=8,310, 95% CI: 3,415-20,220), history of hypertension (OR=2,454, 95% CI: 1.159-5,196), history of chronic kidney disease (OR=10,460 times, 95% CI: 3.282-33,331) history of cancer (OR=16,137, 95% CI: 2,798-96,147) in COVID-19 patients who were hospitalized at Haji Adam Malik General Hospital Medan in 2020
