Abstrak:
Human Factor Analysis and Classification System Framework merupakan suatu metode yang telah banyak digunakan untuk investigasi kecelakaan diberbagai bidang seperti penerbangan, pertambangan, kereta api, dan industri lainnya. Metode HFACS terdiri dari beberapa lapisan seperti organizational influences, unsafe leadership, precondition to unsafe act dan unsafe act. Di PT X sendiri belum pernah dilakukan investigasi kecelakaan menggunakan metode ini. Oleh karena itu penulis mencoba melakukan analisis kecelakaan di PT X yang terjadi selama tahun 2014. Dari hasil analisis yang dilakukan diketahui bahwa setiap level pada lapisan HFACS memiliki kelemahan-kelemahan yang perlu diperbaiki untuk memperkuat pertahanan. Unsafe act terdiri dari error dan violation. Kelemahan yang terdapat dalam error berupa skill based error yaitu kegagalan pengoperasian dan kontrol yang tidak benar, kesalahan navigasi dan persiapan dan pemeliharaan alat yang tidak memadai. Untuk Decision error yang menjadi kelemahan adalah berupa prosedur manuver yang tidak benar, gagal mengenali kondisi berbahaya dan penempatan peralatan dan material yang tidak benar. Perceptual error memiliki kelemahan berupa kesalahan memperkirakan jarak, ketinggian, kedalaman, berat dan kondisi permukaan. Sedangkan yang menjadi kelemahan pada Violation adalah berupa tingginya pelanggaran SOP, kegagalan mengamankan peralatan dan pelanggaran batas kecepatan. Kelemahan pada precondition to unsafe act yang terdiri dari environmental factor, condition of employee serta personal factor yaitu environmental factor berupa kondisi cuaca yang buruk, kondisi jalan tambang yang berbahaya, jalanan licin serta peralatan dan tools yang rusak. Sedangkan untuk condition of operator yaitu kurangnya kompetensi. Untuk personal factor yang menjadi kelemahan adalah komunikasi dan koordinasi bahaya yang buruk. Pada level unsafe leadership yang menjadi kelemahan terbesar terdapat pada elemen inadequate leadership yaitu berupa pengawasan pekerjaan yang tidak memadai serta kegagalan melakukan penilaian risiko. Level terakhir pada HFACS yaitu organizational influences dimana pada level ini yang menjadi kelemahan adalah berupa pelaksanaan kebijakan yang tidak konsisten serta manajemen feedback dari karyawan yang tidak memadai. Abstrak:
Tugas penanganan pasien yang dilakukan oleh perawat seringkali menyebabkan gangguan muskuloskeletal terhadap perawat, dalam beberapa kasus didapati bahwa banyak perawat yang mengalami Low Back Pain (LBP) karena kegiatan manual handling dalam penanganan pasien. Penelitian ini dilakukan untuk menganalisa hubungan faktor manusia dan organisasi dengan keluhan LBP pada perawat di Rumah Sakit A, Kota B menggunakan pendekatan Human Factor Classification Analysis System. Desain penelitian yang digunakan adalah studi cross-sectional, dengan uji chi-square sebagai uji statistik yang digunakan, serta pengumpulan data dilakukan dengan menggunakan metode self reported/administered melalui kuesioner. Hasil penelitian ini menemukan adanya hubungan yang signifikan antara faktor olahraga, pendidikan, dan masa kerja terhadap kejadian LBP dengan masing-masing nilai p=0,019; 0,038; dan 0,04. Selain itu hasil penelitian juga menemukan bahwa dari empat dimensi dalam variabel unsafe supervision terdapat dua dimensi yang memiliki hubungan signifikan terhadap kejadian LBP yaitu, dimensi inadequate supervision (p=0,002), dan dimensi supervisory violation (p=0,047). Sedangkan dari lima dimensi pada variabel precondition for unsafe acts, terdapat satu dimensi yang memiliki hubungan signifikan terhadap kejadian LBP yaitu, dimensi adverse mental states (0,016). Dan dari lima dimensi pada variabel unsafe acts, terdapat dua dimensi yang memiliki hubungan signifikan terhadap kejadian LBP yaitu, dimensi skill based error (p=0,007) dan routine violation (p=0,043). Sebagai kesimpulan, berdasarkan hasil analisis menggunakan pendekatan HFACS, setiap kegagalan dari keempat barier pencegah terjadinya insiden, tiga diantaranya memiliki hubungan yang signifikan terhadap keluhan LBP. Abstrak:
PT. XY perusahaan jasa pelayanan Dump Truck anak perusahaan semen di sektor tambang batu kapur, di PT. XY pernah terjadi kecelakaan kerja maka analisis kecelakaan dengan metode HFACS-MI perlu dilakukan. Penelitian ini bertujuan untuk menganalisis faktor-faktor yang berkontribusi latent dan active failures menyebabkan kecelakaan kerja pengoperasian dump truck. Pada penelitian ini menggunakan desain penelitian studi kasus dengan metode semi kuantitatif dengan pendekatan deskriptif. Populasi dalam penelitian ini adalah data laporan kecelakaan berupa hasil investigasi dari pengoperasian DT sebanyak 27 kasus kecelakaan kerja tahun 2019-2021. Hasil penelitian menunjukkan bahwa kategori dari HFACS-MI yang paling besar berkontribusi adalah organizational influences sebanyak 429 terkait dengan kurangnya analisis keselamatan kerja. Kemudian disusul unsafe leadership sebanyak 370 terkait dengan pengawasan pekerjaan yang tidak memadai. Precondition for unsafe act sebanyak 289 terkait dengan kondisi permukaan jalan licin. Unsafe act sebanyak 247 terkait dengan kegagalan mengenali bahaya. Dan yang paling kecil kategori berkontribusi ialah outside factor sebanyak 1 terkait dengan workshop diluar perusahaan. Disimpulkan bahwa framework HFACS-MI pada latent failures yang banyak berkontribusi ialah organizational influences dan pada active failures yang banyak berkontribusi ialah unsafe act, maka saran tindakan perbaikan di tiap kategori HFACS-MI pada perbaikan latent dan active failures dengan penekanan pada kategori organizational influences.

This research to analyze the factors that contribute laten and active failures to accidents in dump truck operations based on the HFACS-MI framework. This research uses a case study research design with a semi-quantitative method with a descriptive approach. The population in this study is accident report data in the form of investigation results from the operation of DT as many as 27 cases of work accidents in 2019-2021. Abstrak:
Tesis ini membahas studi kasus kecelakaan kerja akibat gas beracun di tambang bawah tanah PT Freeport Indonesia. Penelitian ini adalah penelitian desain deskriptif dan analisis dilakukan menggunakan metode Human Factor Analysis and Classification System (HFACS). Hasil penelitian mendapatkan bahwa terdapat 3 kasus kecelakaan akibat gas beracun di seluruh blok penambangan bawah tanah PTFI selama periode 2019-2022 dengan faktor risiko berasal dari tindakan tidak aman yang dikategorikan dalam Human Factor Analysis and Classification System (HFACS) yang terbagi menjadi empat yaitu organizational influences, unsafe leadership, precondition for unsafe acts, dan unsafe acts. Pada penelitian didapatkan hasil kegagalan sistem pertahanan pada pengaruh organisasi dalam kasus keracunan gas beracun di dominasi kategori resource management berjumlah 7 kegagalan (57%), faktor-faktor penyumbang kegagalan sistem manajemen antara lain SOP, safety sign, Planned Inspection, PJO, Kebijakan K3, Data base incident management system dan komunikasi saat penyampaian instruksi kerja. Tingkat Unsafe Supervision dalam kasus keracunan gas beracun berjumlah 11 kegagalan (55%) dengan dominasi oleh kategori Supervisory Violation, faktor-faktor penyumbang kegagalan sistem manajemen antara lain Neil George Checklist, Bulkhead Ventilation, Re-entry Checklist, Airlock Door, Vent bag, Supervisor Inspection, Alat Pelindung Diri, Pengawasan di lapangan, kelayakan sistem ventilasi di lokasi kerja, observasi pengawas ke pekerja terkait pengoperasian portable gas detector dan instruksi kerja. Tingkat Precondition for Unsafe Acts dalam kasus keracunan gas beracun didominasi kategori Personal Readiness berjumlah 8 kegagalan (38%), faktor-faktor penyumbang kegagalan sistem manajemen antara lain peralatan blasting, PDA, pemahaman bahasa pengantar, emission test alat berat, ceklist stopevent dan ventilasi di area blasting, tidak mempedulikan alarm gas detector. Tingkat Unsafe Acts dalam kasus keracunan gas beracun berjumlah 11 kegagalan (46%) dari dominasi kategori Violation Routine, faktor-faktor penyumbang kegagalan sistem manajemen antara lain chemical handling, prosedur yang tidak memadai, sign nilai ambang batas dalam satu bahasa, dumper vent terhalang lumpur dan vent bag rusak, fixed gas detector belum terkalibrasi dan tertutup lumpur, evaluasi pelatihan tidak konsisten dilakukan, barikade area, tidak mengikuti re-entry protocol. Perusahaan disarankan untuk melakukan evaluasi pada program penanganan kecelakaan akibat gas beracun.

The results of the study found that there were 3 cases of accidents due to toxic gas in all PTFI underground mining blocks during the 2019-2022 period with risk factors originating from unsafe actions which were categorized in the Human Factor Analysis and Classification System (HFACS), which was divided into four, namely organizational influences, unsafe leadership, precondition for unsafe acts, and unsafe acts. In the study, the results of the failure of the defense system on organizational influence in cases of poison gas poisoning were dominated by the category of resource management totaling 7 failures (57%), contributing factors to the failure of the management system including SOP, safety sign, Planned Inspection, PJO, K3 Policy, Database incident management system and communication when submitting work instructions. . The level of Unsafe Supervision in cases of poison gas poisoning amounted to 11 failures (55%) with dominance by the Supervisory Violation category, contributing factors to management system failure including the Neil George Checklist, Bulkhead Ventilation, Re-entry Checklist, Airlock Door, Vent bag, Supervisor Inspection, Personal Protective Equipment, Supervision in the field, feasibility of the ventilation system at the work site, supervisor's observation of workers regarding the operation of portable gas detectors and work instructions. The level of Precondition for Unsafe Acts in cases of poison gas poisoning was dominated by the Personal Readiness category with 8 failures (38%), contributing factors to management system failure including blasting equipment, PDA, understanding of the language of instruction, emission test of heavy equipment, stop vent checklist and ventilation in the blasting area, ignoring the gas detector alarm. The level of Unsafe Acts in cases of poison gas poisoning amounted to 11 failures (46%) from the dominance of the Violation Routine category, contributing factors to management system failure including chemical handling, inadequate procedures, sign threshold values in one language, dumper vents blocked by mud and the vent bag is damaged, the fixed gas detector has not been calibrated and is covered in mud, inconsistent training evaluations are carried out, barricaded areas, do not follow the re-entry protocol. Klub Motor X merupakan salah satu klub motor yang anggotanya tersebar di seluruhIndonesia, namun untuk penelitian kali ini hanya dibatasi wilayah Jabodetabek saja.Klub motor ini memiliki agenda rutin yaitu melakukan turing. Dalam perjalananturing tersebut, ditemukan berbagai risiko yang dapat menimbulkan terjadinyakecelakaan. Meski tidak tercatat secara detail, diketahui telah ada kasus kecelakaanbahkan sampai memakan jiwa selama yang terjadi selama perjalanan turing.Penelitian ini dilakukan untuk mengetahui gambaran risiko apa saja yang dapatmenyebabkan terjadinya kecelakaan dengan menggunakan metode Human FactorAnalysis Classification System (HFACS). Kemudian permasalahan terkait gegagalandari sistem pertahanan HFACS yang banyak ditemukan adalah adversephysiological states, adverse mental states, personal readiness, routine violations,dan perceptual errors. Human Factors Analysis and Classification System in Mining Industry (HFACS-MI) merupakan suatu metode investigasi kecelakaan untuk mencari faktor-faktor penyebab kecelakaan pada industri pertambangan. Metode HFACS sendiri telah banyak digunakan untuk investigasi kecelakaan diberbagai industri seperti penerbangan, konstruksi, kereta api, dan industri lainnya. Metode ini terdiri dari 5 (lima) tingkatan yaitu unsafe act, precondition for unsafe act, unsafe leadership, organizational influences, dan outside factor. PT. XYZ merupakan salah satu perusahaan pertambangan di wilayah Kalimantan Timur. Kecelakaan yang sudah terjadi tentunya membuat perusahaan mengalami kerugian, perlunya dilakukan kajian proses analisis secara detail untuk mengetahui faktor penyebab kecelakaan bersifat aktif dan laten serta mengetahui keterkaitan penyebab kecelakaan dari berbagai tingkat dengan menggunakan metode HFACS-MI.

Kebakaran merupakan salah satu jenis kecelakaan kerja yang umum terjadi di sektor manufaktur, termasuk industri semen. PT X merupakan salah satu pabrik semen yang melaporkan 21 kasus kebakaran dari tahun 2020 hingga 2024, yang mengindikasikan tren kebakaran berulang setiap tahunnya. Penelitian ini bertujuan untuk menganalisis tren kebakaran di PT X dari perspektif faktor manusia (human factors), melalui pendekatan Human Factors Analysis and Classification System (HFACS). Dengan penggunaan metode kualitatif berbasis desain deskriptif analitik, penelitian ini mengandalkan data sekunder dari laporan investigasi insiden perusahaan dan wawancara dengan pihak terkait. Analisis data dilakukan untuk menjelaskan karakteristik dan faktor penyebab yang mendasari tren kebakaran berdasarkan kerangka HFACS. Hasil penelitian menemukan bahwa kondisi laten lebih mempengaruhi tren kebakaran secara signifikan dibandingkan kegagalan aktif. Faktor penyebab utama yang teridentifikasi dari kondisi laten meliputi resource management, inadequate supervision dan crew resource management, sedangkan decision errors merupakan faktor penyebab dominan yang teridentifikasi dari kegagalan aktif.
Fire incidents are a prevalent type of work-related accident in the manufacturing sector, including the cement industry. PT X is one of the cement factories that reported 21 fire cases from 2020 to 2024, indicating a concerning trend of recurring incidents yearly. This study aims to analyze the fire trend at PT X through the lens of human factors, utilizing the Human Factors Analysis and Classification System (HFACS) approach. Employing a qualitative method with a descriptive-analytical design, the research relies on secondary data from the company's incident investigation reports and interviews with relevant stakeholders. This study performed data analysis to elucidate the characteristics and causal factors underlying the trend in fire incidents, utilizing the Human Factors Analysis and Classification System (HFACS) framework. The findings reveal that latent conditions significantly impact the fire trend more than active failures. Key causal factors identified in latent conditions include resource management, inadequate supervision, and crew resource management, while decision errors are the dominant causal factors identified in active failures.

This study aims to analyze the fire trend at PT X through the lens of human factors, utilizing the Human Factors Analysis and Classification System (HFACS) approach. Employing a qualitative method with a descriptive-analytical design, the research relies on secondary data from the company's incident investigation reports and interviews with relevant stakeholders. This study performed data analysis to elucidate the characteristics and causal factors underlying the trend in fire incidents, utilizing the Human Factors Analysis and Classification System (HFACS) framework. The findings reveal that latent conditions significantly impact the fire trend more than active failures. Key causal factors identified in latent conditions include resource management, inadequate supervision, and crew resource management, while decision errors are the dominant causal factors identified in active failures.

Sebagian besar kegiatan kerja dalam logistik adalah pendistribusian barang atau bahan produksi dengan menggunakan kendaraan berupa truk yang berhubungan dengan keselamatan transportasi jalan raya. Penelitian ini memberikan gambaran analisis kecelakaan transportasi truk berdasarkan data KNKT tahun 2021–2022 dengan menggunakan desain penelitian analisis deskriptif dan mengadaptasi metode analisis kecelakaan Human Factors Analysis and Classification System (HFACS). Penelitian ini menggunakan data sekunder dalam pengumpulan data, yaitu berupa laporan investigasi kecelakaan transportasi truk berdasarkan data KNKT tahun 2021–2022. Hasil penelitian ini diketahui bahwa yang memiliki jumlah kejadian terbanyak atau lubang terbesar, yaitu dalam kegagalan aktif adalah skill-based errors, sedangkan dalam kondisi laten adalah resource management, technological environment, dan inadequate supervision.
Most work activities in logistics are the distribution of goods or production materials using vehicles in the form of trucks related to road transportation safety. This study provides an overview of the analysis of truck transportation accidents based on KNKT data for 2021–2022 using a descriptive analysis research design and adapting the Human Factors Analysis and Classification System (HFACS) accident analysis method. This study uses secondary data in data collection, namely in the form of truck transportation accidents investigation reports based on KNKT data for 2021–2022. The results of this study show that those with the highest number of incidents or the largest holes, namely in active failure are skill-based errors, while in latent conditions are resource management, technological environment, and inadequate supervision.

Industri pertambangan dikenal sebagai lingkungan kerja dan aktivitas operasional dengan risiko tinggi, bahkan dikategorikan sebagai salah satu aktivitas paling berbahaya pada industri modern. Industri ini berada di antara peringkat tertinggi yang mengalami cedera dan hari kerja hilang, terdapat banyak jenis cedera yang terjadi dan cedera adalah salah satunya. Jari dilaporkan menjadi salah satu bagian tubuh yang paling sering mengalami cedera. Penelitian ini dilakukan pada salah satu perusahaan tambang di Indonesia, PT X, dalam jangka waktu lina tahun (2018-2022) menggunakan data hasil investigasi kecelakaan. Dalam periode ini data statistik menunjukkan adanya tren peningkatan dan berdasarkan hasil investigasi kecelakaan menggunakan metode RCA ditemukan bahwa 74% kasus akibat dari perilaku atau kegagalan manusia. Analisis lebih mendalam dilakukan menggunakan kerangka kerja HFACS-MI untuk mengetahui faktor penyebab terjadinya kegagalan dalam berbagai lapisan pertahanan di dalam perusahaan. Faktor penyebab terbesar dibandingkan dengan jumlah kasus adalah kategori Organizational Influences (196,17%) dikuti oleh Unsafe Acts (156,28%), Precondition for Unsafe Acts (117,49%), dan Unsafe Leadership (97,27%).
Mining industries are known as high-risk working environments and operational activities, they are categorized as one of the most dangerous activities in modern industry today. It ranks among the highest producers of injuries and lost working days, there are many types of injuries that contribute to this, finger injury is one of them. Fingers are reported to be one of the most injured body parts. This research carried out in a mining company in Indonesia, PT X, within a period of five years (2018-2022) based on their accident investigation data. Within this period statistical data shows an increasing trend and referring to accident investigation conducted using RCA method it was found that 74% due to behavior or human error. Further analysis conducted using HFACS-MI framework to find out causal factors contributing to failure in various defence layer within organization. The biggest contributing factors, compared to number of cases, are Organizational Influences category (196.17%) followed by Unsafe Acts (156.28%), Precondition for Unsafe Acts (117.49%), and Unsafe Leadership (97.27%).

PT BCY, sebagai perusahaan yang bergerak di bidang pengeboran dan kerja ulang sumur minyak, memiliki risiko tinggi terhadap kecelakaan kerja. Data dari departemen Healthy Safety and Environment (HSE) PT BCY menunjukkan bahwa selama periode 2019–2024, terjadi beberapa insiden kecelakaan kerja yang menyebabkan cedera serius hingga fatal. Kinerja Keselamatan PT BCY Sepanjang 2019-2024 tercatat terdapat 26 kasus, perusahaan telah menerapkan standar keselamatan kerja. Menganalisis faktor-faktor yang memengaruhi kecelakaan kerja di PT BCY antara tahun 2019 – 2024 dengan metode HFACS.Penelitian ini menggunakan metode penelitian deskriptif analitik dengan metode gabungan. Data sekunder yang digunakan berupa rekaman kejadian kecelakaan dan laporan investigasi atas 26 kejadian kecelakaan dan hampir celaka di PT BCY. Data sekunder tersebut kemudian diklasifikasikan sesuai dengan empat (4) tahapan kegagalan di metode HFACS, yaitu unsafe acts, precondition of unsafe acts, unsafe supervision, dan organizational influence. Data primer melalui kuesioner kemudian dinalisis menggunakan uji bivariat untuk memvalidasi persepsi responden terhadap temuan dari data sekunder. Berdasarkan analisis HFACS, faktor Preconditions for Unsafe Act merupakan yang paling banyak teridentifikasi (77 identifikasi), diikuti Unsafe Acts (60 identifikasi), Organizational Influences (38 identifikasi), dan Unsafe Leadership (36 identifikasi). Disimpulkan Faktor yang berhubungan dengan kecelakaan kerja adalah Organizational Influences: Resources Management, Organization Culture, Organizational Process, pada Faktor Unsafe Leadership: Inadequate Leadership, Planned Inappropriate Operations, Failed to correct problem, pada faktor Preconditions for Unsafe Act: Lingkungan Fisik, Lingkungan Teknik, Kondisi Mental, Kondisi Fisiologis, Keterbatasan Fisik, Kesiapan Pekerja, untuk Faktor Unsafe Acts: Skill based error, Decision Error, dan Routine Violations. Sedangkan Faktor yang tidak berhubungan signifikan adalah Leadership Violations, Kegagalan Mengelola Pekerjaan, dan Exceptional Violations.
PT BCY, as a company operating in oil well drilling and workover, carries a high risk of work related accidents. Data from PT BCY's Health, Safety, and Environment (HSE) department indicates that during the 2019–2024 period, there were several work related accident incidents leading to serious and even fatal injuries. PT BCY's safety performance throughout 2019-2024 recorded 26 cases despite the company having implemented work safety standards. The objective of this research was to analyze the factors influencing work related accidents at PT BCY between 2019 and 2024 using the Human Factor Analysis and Classification System (HFACS) method. This study employed a descriptive analytic research design with a mixed-methods approach. Secondary data consisted of accident records and investigation reports for 26 accident and near-miss incidents at PT BCY. This secondary data was then classified according to the four (4) stages of failure in the HFACS method: unsafe acts, preconditions for unsafe acts, unsafe supervision, and organizational influences. Primary data was collected through questionnaires and then analyzed using bivariate tests to validate respondents' perceptions against the findings from the secondary data. Based on the HFACS analysis, "Preconditions for Unsafe Act" was the most frequently identified factor (77 identifications), followed by "Unsafe Acts" (60 identifications), "Organizational Influences" (38 identifications), and "Unsafe Leadership" (36 identifications). It was concluded that the factors significantly related to work accidents were: For "Organizational Influences": Resource Management, Organization Culture, and Organizational Process. For "Unsafe Leadership": Inadequate Leadership, Planned Inappropriate Operations, and Failed to Correct Problem. For "Preconditions for Unsafe Act": Physical Environment, Technical Environment, Mental Condition, Physiological Condition, and Personnel Readiness.
