ABC Hospital Jakarta, which began to have an agreement with BPJS HealthInsurance in May 2017, saw the need to recruit full timer specialists to support specialistservices in the NHC era. With the absence of a good allowance system at ABC HospitalJakarta, it was necessary to have a design on remuneration for medical specialist as anallowance system at ABC Hospital Jakarta in the NHC era.The research method used was a combination of quantitative and qualitative studyusing primary and secondary data. Primary data obtained from in-depth interviews andsurveys, while secondary data was taken from the results of document review. This studyresulted in the design of a remuneration system for specialists adapted from the Ministerof Health Decree No. 625/Menkes/SK/V/2010 and IDI Remuneration Guidebook of2016, which taken into account the principles of remuneration and three components ofremuneration (Pay for Position, Pay for Performance and Pay for People).Keywords : remuneration design, remuneration for doctor, remuneration in NHC era,remuneration in private hospital, principles of remuneration, component ofremuneration.
Penelitian ini dilatarbelakangi oleh adanya perbedaan komitmen diantara dokter spesialis di rumah sakit BaliMed yang sebagian besar adalah pemilik saham, dalam upaya meningkatkan kualitas dan kuantitas pelayanan. Kuantitas ditinjau dari jumlah rujukan dan jumlah pasien yang dirawat di rumah sakit, sedangkan kualitas ditinjau dari kedisiplinan dokter spesialis dalam menjamin mutu pelayanan melalui ketepatan waktu praktek, visite dan kelengkapan administrasi dalam pelayanan, dokter spesialis adalah ujung tombak pelayanan di rumah sakit. Pengaruh peran manajerial yaitu Kepemimpinan, Kompensasi dan Iklim kerja terhadap komitmen dokter spesialis kemudian dinilai ditinjau dari sudut persepsi dokter spesialis yang melakukan praktek di rumah sakit BaliMed. Penelitian ini merupakan penelitian kuantitatif dengan disain cross sectional. Hasil analisis bivariat memperlihatkan bahwa ada hubungan yang bermakna antara peran manajerial dengan komitmen dokter spesialis antara lain Kepemimpinan (p=0, 00), Kompensasi ( p = 0, 00 ) dan Iklim kerja (p= 0, 00 ). Hasil Analisis Multivariat menggunakan analisis regresi linier berganda dengan alpha < 0, 05 menunjukkan bahwa secara bersama-sama Peran manajerial Kepemimpinan, Kompensasi dan Iklim kerja berpengaruh secara bermakna terhadap Komitmen Dokter spesialis (p=0, 000). Sedangkan secara terpisah variabel kepemimpinan(0, 990) tidak berpengaruh terhadap komitmen dokter sedangkan faktor kompensasi( p= 0, 024) dan iklim kerja (p=0, 000) mempunyai pengaruh yang bermakna terutama iklim kerja mempunyai pengaruh paling besar terhadap komitmen dokter spesialis. This research was performed since there was a diferrence in regards of commitment among consulting doctors especially the specialists in BaliMed Hospital, whereas most of them are owners of the hospital, in terms of increasing the quality and quantity of service. Quantity is the volume of patients reffered and hospitalized in the hospital, while quality is the discipline of the specialists in terms of punctuality of their schedule and the administration completeness. These are very important since specialist is the spearhead of hospital service.The influence of managerial roles, such as Leadership, Compensation, and Work Climate to the specialists’ commitments observed from the specialists who practice at BaliMed Hospital. This research is a quantitative with cross sectional design. Bivariate analysis result shows that there is significant correlation between Managerial Role with Commitment of specialists such as Leadership (p=0.00), Compensation (p=0.00), dan Work Climate (p=0.00). Multivariate analysis result using multiple linear regression analysis with alpha < 0.05 shows that Managerial Role, Leadership, Compensation, dan Work Climate simultaneously have a significant influence to the specialist commitment (p=0.000). In the other hand, leadership variable (p=0.990) solely has no influence with specialist commitment, while compensation (p=0.024 and especially work climate (p=0.000) have a significant influence to the commitment of the specialist. Sharia hospital is a hospital that carries out all activities, both patient care and management based on the principles of Maqashid Al-Shari'ah. This study aims to determine the relationship between quality indicators and minimum service standards of Sharia Hospital on the performance of Medical Check-Up (MCU) services at YARSI Hospital Jakarta. The design used in this research is a mixed methods research in a cross-sectional way. From the results of the study, it was found that the compliance of officers in identifying patients, compliance by officers in washing hands 6 steps 5 moments, hijab (veil, patient clothes, or cloth) for patients, installation of an ECG according to gender, reminding patients to pray, and gharar (uncertainty) had a relationship with the performance of MCU services after Sharia certification at YARSI Hospital Jakarta in the form of extending MCU service time, preventing infection to MCU patients, preventing contamination, reducing doubts in action, being on time, and no parties feeling aggrieved. It is recommended to YARSI Hospital Jakarta, especially at the MCU installation so that the flow of MCU patient care is followed by all MCU officers, regular briefings for uniformity of MCU services, and giving rewards/punishments to MCU officers.
Kepatuhan adalah sikap mentaati peraturan dan ketentuan yang telah ditetapkan tanpa pamrih. Rendahnya tingkat kepatuhan dokter spesialis terhadap kebijakan dan aturan di rumah sakit penting untuk segera ditangani oleh manajemen. Tujuan penelitian obsevasional ini, dengan pendekatan rancangan penelitian cross sectional, adalah mengetahui faktor-faktor internal dan eksternal yang mempengaruhi kepatuhan dokter spesialis terhadap kebijakan dan aturan di RS Hermina Daan Mogot.
Populasi penelitian: seluruh dokter spesialis di RS Hermina Daan Mogot. Analisis statistik yang digunakan: analisis bivariat dengan uji Chi Squre. Uji statistik Chi Square menghasilkan variabel yang memiliki hubungan bermakna dengan kepatuhan dokter spesialis yaitu p-value ≤ 0,05 adalah motivasi (p-value = 0,043). Dari analisis diperoleh pula nilai OR=2,4, artinya dokter yang motivasinya lebih mempunyai peluang 2,4 kali lebih baik dalam kepatuhan dibanding dokter yang motivasinya kurang. Perlu upaya mempertahankan motivasi kerja para dokter agar tetap di level yang tinggi, sehingga akhirnya berdampak positif bagi perkembangan rumah sakit kedepan.
Compliance is the attitude of obeying the rules and conditions set unconditionally. Low levels of compliance specialist in the policies and rules of the hospital is important to be addressed by management. The purpose of this observational research, with cross-sectional study design approach, is to know the internal and external factors affecting the specialists adherence to policies and rules in Hermina Daan Mogot Hospital.
Study population: all specialists in Hermina Daan Mogot Hospital. Staristical analysis used: bivariate analysis with Chi squre test. Chi Square statistical test results in variables that have a significant relationship with compliance specialists where p-value ≤ 0.05 is motivation (p-value = 0.043). Obtained also from the analysis of the value of OR = 2.4, meaning that physicians with more motivation have greater odds 2.4 times better adherence than physicians with less motivation. Necessary efforts are to maintain the motivation of doctors to remain at a high level, so that ultimately have a positive impact on the future development of the hospital.
