Background: Health Statistics in 2023 recorded that the maternal mortality rate was 194 out of 100,000 KH, while the government set a target of only 70 out of 100,000 KH, which means that the reduction in maternal mortality is still far from the government's target. The 2021 Health Service Decree regarding e-Cohort states that to reduce the number of KIA deaths, it is necessary to strengthen governance by improving the online recording system. The 2019 Technical Guidelines explain that e-Cohorts are an important instrument for identifying information related to the health conditions of mothers and babies regarding the availability of accurate data for the program planning and evaluation process. In line with Rani's (2014) research, the use of e-Cohort in the process of recording pregnant women, birthing mothers, postpartum mothers and babies is an effort by the government to reduce maternal and child mortality. If this is not done, it will have an impact on the scope of recording which is not carried out early. A preliminary study conducted at the Health Service found differences in recording in the e-Cohort application so researchers were interested in seeing an overview of the e-Cohort evaluation to increase ANC coverage for pregnant women in Cilegon City in 2023 using the HOT-Fit Model method. Objective: To understand the picture of increasing ANC coverage based on e-Cohort. Method: This research uses a descriptive method with a cross-sectional approach using a survey. Results: The results of a simple analysis of the questionnaire instrument show that the average results regarding the benefits of using e-Cohort are 65% which is included in the High category. The most prominent indicator is the ease of use for users in using e-Cohort at 75%. Meanwhile, the lowest is the usefulness of e-Cohorts in decision making which is linked to the form of e-Cohort reports which are still limited. So it can be a top priority for improvements that must be made to increase the benefits of using e-Cohort. Conclusion: Respondents rated the Human variable as still dissatisfied because they felt that e-Cohort did not make the user's work easier and did not make the work completed more quickly; Variable: The respondent's organization is quite satisfied because of the support from the leadership for the implementation of the transition from PWS to e-Cohort which was stated in the Decree before the implementation of e-Cohort; The Technology variable is a variable that many respondents assess less because the information produced is not as ideal when using PWS and the IT team is less responsive in answering problems that are obstacles; The Net Benefit variable produced by the e-Cohort application is felt by respondents to be lacking in providing effectiveness and efficiency in the work they do.
